What you'll be doing:
You will be a welcomed member of the Norfinch Community team and impact in the following ways:
- Provide sound clinical knowledge and skills, acting as clinical expert and resource
- Provide comprehensive, quality resident and family-centered health care services
- Aide in the transition of long term care residents with responsive behaviours between community, hospital and neighboring long-term care communities (LTCHs)
- Consult on resident applications and make recommendations to meet the needs related to responsive/expressive behaviours
- Screen and complete risk assessment for residents with new or change in behaviours
- Coordinate referrals and engage community partners in supporting the resident care needs
- Facilitate education sessions to build capacity in team members
- Support the management of case reviews and clinical rounds
- Engage the interprofessional team in the development of individualized plan of care
- A Registered Practical Nurse in good standing the Ontario College of Nurses
- Minimum 3 years' experience working in long term care
- Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions
- Specialized training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Montessori, DementiAbility are an asset
- You have good judgement, the ability to identify risk and can foster a positive work environment
