Reporting to the Director of Care, the Registered Practical Nurse - Behavioural Supports Nurse is a critical role within our communities as it impacts the lives of residents.

What you'll be doing:
You will be a welcomed member of the Norfinch Community team and impact in the following ways:
  • Provide sound clinical knowledge and skills, acting as clinical expert and resource
  • Provide comprehensive, quality resident and family-centered health care services
  • Aide in the transition of long term care residents with responsive behaviours between community, hospital and neighboring long-term care communities (LTCHs)
  • Consult on resident applications and make recommendations to meet the needs related to responsive/expressive behaviours
  • Screen and complete risk assessment for residents with new or change in behaviours
  • Coordinate referrals and engage community partners in supporting the resident care needs
  • Facilitate education sessions to build capacity in team members
  • Support the management of case reviews and clinical rounds
  • Engage the interprofessional team in the development of individualized plan of care
Who you are:
  • A Registered Practical Nurse in good standing the Ontario College of Nurses
  • Minimum 3 years' experience working in long term care
  • Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions
  • Specialized training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Montessori, DementiAbility are an asset
  • You have good judgement, the ability to identify risk and can foster a positive work environment
