The Registered Practical Nurse (RPN) plays a key role in supporting the healthcare team by conducting comprehensive assessments for PSW clients under the supervision of a Registered Nurse (RN). The primary responsibilities include visiting clients in their homes, hospitals, or other healthcare settings to evaluate their environment and overall well-being. The RPN will perform client assessments, develop personalized care plans, and collaborate with interdisciplinary team members to ensure the delivery of high-quality care and positive client outcome

The Registered Practical Nurse (RPN) plays a key role in supporting the healthcare team by conducting comprehensive assessments for PSW clients under the supervision of a Registered Nurse (RN). The primary responsibilities include visiting clients in their homes, hospitals, or other healthcare settings to evaluate their environment and overall well-being. The RPN will perform client assessments, develop personalized care plans, and collaborate with interdisciplinary team members to ensure the delivery of high-quality care and positive client outcome
Responsibilities
- Performs client assessment in the community
- Provides accurate, detailed, and timely documentation
- Conducts ongoing assessments
- Create plan of care
- Prepares, administers and appropriately documents actions taken specific to medications
- Performs all skills, treatments, and procedures competently in accordance with the provincial scope of practice
- Follows principles of infection control and Universal Precautions
- Current certificate of competence and good standing
- Current CPR/First Aid Certification.
- Clear Police Record Check (Vulnerable Sector Screening) or willingness to obtain one.
- Negative TB test or chest X-ray
- Strong clinical skills
- Self-directing with the ability to work with little direct supervision
- Excellent customer service skills
- Reliable internet access and a mobile phone.
- Experience with Dementia/Alzheimer's care is an asset.
