Your Opportunity:
As a HFHT Mental Health RN (Intake & Triage Role), you will be completing individual assessments, developing care plans and increasing access to direct and indirect consultation for HFHT patients whose family physicians do not have on-site psychiatry consultation. Experience in motivational interviewing and additional languages are considered assets.

Roles and Responsibilities:
- Triaging referrals
- Gathering relevant history from the practice, other providers, and family, including that relevant to both physical and mental health
- Participate in the psychiatric assessments as needed
- Collaborate with practices to ensure follow-through on recommendations
- Collaborating with community resources to provide system navigation
- Providing data that will be used for planning purposes
- Under the supervision of the psychiatrists, support education of patients, families, and other providers re: pharmacological treatment
- Maintain professional competence through participation in professional development activities and maintaining license to practice
- Comply with all relevant HFHT policies, procedures, and protocols
- Other duties as required
Qualifications and Skills:
- Five years of experience working as a RN in adult mental health, CPMHN(C) or equivalent and membership in professional college
- Strong organizational and communication skills
- Honed documentation skills
- Ability to collaborate with other providers, support education and capacity building
- Ability to understand and respond to the social determinants of health and impact on mental illness and well-being
- Registration with the College of Nurses of Ontario as an RN
- High level of competency with computers and information technology
- Respects and values the diversity of communities and individuals
