Your Opportunity:

As a HFHT Mental Health RN (Intake & Triage Role), you will be completing individual assessments, developing care plans and increasing access to direct and indirect consultation for HFHT patients whose family physicians do not have on-site psychiatry consultation. Experience in motivational interviewing and additional languages are considered assets.

The Hamilton Family Health Team is seeking a Registered Mental Health RN 0.8 FTE (32 hours per week) position. The Hamilton Family Health Team is actively committed to creating a diverse and inclusive workplace and encourages applications from all qualified candidates.

Roles and Responsibilities:
  • Triaging referrals
  • Gathering relevant history from the practice, other providers, and family, including that relevant to both physical and mental health
  • Participate in the psychiatric assessments as needed
  • Collaborate with practices to ensure follow-through on recommendations
  • Collaborating with community resources to provide system navigation
  • Providing data that will be used for planning purposes
  • Under the supervision of the psychiatrists, support education of patients, families, and other providers re: pharmacological treatment
  • Maintain professional competence through participation in professional development activities and maintaining license to practice
  • Comply with all relevant HFHT policies, procedures, and protocols
  • Other duties as required


Qualifications and Skills:
  • Five years of experience working as a RN in adult mental health, CPMHN(C) or equivalent and membership in professional college
  • Strong organizational and communication skills
  • Honed documentation skills
  • Ability to collaborate with other providers, support education and capacity building
  • Ability to understand and respond to the social determinants of health and impact on mental illness and well-being
  • Registration with the College of Nurses of Ontario as an RN
  • High level of competency with computers and information technology
  • Respects and values the diversity of communities and individuals

Accountabilities:

Under the clinical direction of the psychiatrists.

Work collaboratively with the Mental Health Nurse Coordinator for Centralized Child & Youth Psychiatry Services and Manager of Mental Health and Nutrition

Salary: $74,148 (pro-rated to FTE)
