• You are Dependable and can maintain a Consistent Schedule
• You have a valid Driver's License and Reliable Vehicle

• Must be willing to travel within the Uxbridge and Port Perry area

Job Summary
The Personal Support Worker is a member of the home care team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the client's needs.

Responsibilities

• Assistance with personal care and hygiene
• Meal Preparation
• Light Housekeeping
• Accompany clients on outings
• Shopping and errands
• Medication Assist/ Reminders
• Ambulation and transporting of clients
• Provide companionship to clients
• Ability to follow care-plan
• Documentation/Charting

Qualifications

• PSW/DSW/SSW/HCW (or equivalent) certification from an accredited educational institution
• Valid Police Vulnerable Sectors Check
• Current CPR/First Aid Certification
• Negative TB skin test or chest x-ray
• Up-to-date Immunization Record, including two COVID-19 vaccinations
• Valid driver's license and reliable vehicle is required
• Access to Internet/mobile phone
• Must be able to wear Personal Protective Equipment (PPE)
