Do you have what we're looking for?
- You are available between the hours of 8AM-6PM on Monday thru Fridays AND every other weekend
- You are Dependable and can maintain a Consistent Schedule
- You have a valid Driver's License and Reliable Vehicle
- You really Care about people
- You are local to the Port Perry & Uxbridge area OR are willing to drive to Port Perry & Uxbridge
- Must be willing to travel within the Uxbridge and Port Perry area
- Must be ok to see client with pets; such as cats or dogs

Job Summary
The Personal Support Worker is a member of the home care team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the client's needs.
Responsibilities
- Assistance with personal care and hygiene
- Meal Preparation
- Light Housekeeping
- Accompany clients on outings
- Shopping and errands
- Medication Assist/ Reminders
- Ambulation and transporting of clients
- Provide companionship to clients
- Ability to follow care-plan
- Documentation/Charting
Qualifications
- PSW/DSW/SSW/HCW (or equivalent) certification from an accredited educational institution
- Valid Police Vulnerable Sectors Check
- Current CPR/First Aid Certification
- Negative TB skin test or chest x-ray
- Up-to-date Immunization Record, including two COVID-19 vaccinations
- Valid driver's license and reliable vehicle is required
- Access to Internet/mobile phone
- Must be able to wear Personal Protective Equipment (PPE)
