One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a Diabetes Educator (RN) or Registered Dietitian (RD) to deliver diabetes education, health counselling and programming in both individual and group settings. Working from a social determinants of health framework, the Diabetes Educator/Registered Dietitian empowers clients in gaining knowledge, skills and confidence to self-manage their condition. This position is based out of the Thunder Bay site, and will provide outreach to communities served by NorWest CHC Mobile Health Services.

The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.

QUALIFICATIONS:

  • Certified Diabetes Educator or Registered Dietitian designation preferred.
  • Registered Nurse, with current registration in good standing with The College of Nurses of Ontario; OR
  • Registered Dietitian, with current registration in good standing with The College of Dietitians of Ontario.
  • Experience with adult learning principles.
  • Motivational Interviewing training is an asset.
  • Excellent interpersonal, communication, group facilitation and teaching skills.
  • Proven experience developing, delivering and evaluating programs.
  • Ability to work both independently and collaboratively with a multidisciplinary team.
  • Awareness and sensitivity to socio-economic and cultural issues.
  • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQI+, newcomers, non-insured).
  • Knowledge and proficiency in MS Office (MS Excel/MS Word) and electronic medical record systems (PS Suite).
  • Ability to work flexible hours including evenings and weekends.
  • Valid driver's license, access to a vehicle and ability to travel to other service locations.
