TEMPORARY PART-TIME UNTIL APPROX. DECEMBER 2025
Reporting to the Director, Primary Health Care, the Registered Practical Nurse (RPN), Foot Care provides foot care services and safe, effective and ethical nursing care.
As a member of a multidisciplinary team working in collaboration with other health care professionals, the RPN will provide nursing foot care to help clients maintain their highest mobility possible. The RPN assists clients to identify their own health goals, and attain them. The position also includes participation in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.
The position will be based at City designated shelters, e.g., the Homelessness and Housing Help Hub (H4) and the Welcome Centre for Women.
Responsibilities and Duties:
The position responsibilities include the following within the scope of practice guidelines provided by the College of Nurses and under the authorization of a team physician or Nurse Practitioner:
Education and Skills:
Hours of Work: Seventeen and a half (17.5) hour work week. Shift schedules will be determined upon hire. Flexible hours are required to meet client and service needs.
Salary range: Grade 7, $32.85- $38.84 per hour
Per 2022 - 2026 Collective Agreement
Applicable shift premiums are paid in accordance with the Collective Agreement, Article 17.11.

- Perform assessments; provide treatment, health education/counseling and other clinical activities according to the College of Nurses of Ontario (CNO) standards of practice for Registered Practical Nurses (RPN's) as well as working within the scope of practice for RPN's according to CNO.
- Performing foot assessments, developing client-focused treatment plans incorporating health education, culture and income and provides written and oral instructions to patients.
- Ensures foot care resources, materials and equipment are maintained and ordered as required.
- Contribute to chart documentation, participate in chart reviews and case conferences, and make referrals.
- Participate in client injections-long acting psychotropic medication, vitamin B12, MOHLTC funded vaccines.
- Communicable Disease testing and follow up health assessments.
- Management of medication - including receipt and storage of stocked sample medications,
- Conducts detailed intakes for new client e.g. vitals, medical tests, and initial assessment/triage of primary and mental health concerns.
- Medical procedures such as wound care.
- Order/restock clinic supplies.
- Maintenance and quality assurance related to medical equipment and point of care testing;
- Client education - e.g. glucometer teaching, smoking cessation, healthy lifestyles.
- Provider directed client follow-up - e.g. calling clients with test results, referral management;
- Work in consultation with the CMHA Health Centre/CMHA mental health, and health care providers.
- Adhering to standards as set out by the College of Nurses of Ontario and agency Policy and Procedures.
- Following agency procedures and College of Nurses of Ontario Standards regarding documentation of medication orders, medication administration and reporting of errors.
- Other roles and responsibilities as required within the scope of the position.
Education and Skills:
- Registered Practical Nurse diploma. Registered member in good standing with the College of Nurses of Ontario (CNO).
- Advanced Nursing Foot Care Certification is required.
- Two (2) to four (4) years' clinical nursing experience relevant to the provision of primary care.
- At least two (2) years in the provision of foot care nursing.
- Thorough knowledge and proficiency in current nursing physical assessments and treatment methods.
- Ability to analyze and interpret valid research findings in clinical practice.
- Ability to work independently and collaboratively with members of the health care team.
- Knowledge of mental health & relevant legislation.
- Ability to communicate effectively with clients, public, professionals and staff, in written and oral form
- Able to make decisions quickly on available information and take action.
- Bilingualism in both official languages at the advanced level is considered an asset.
- A valid Ontario Driver's License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.
