Virtual Health Concierge – Remote Menopause Care Program (Contract and Full Time)

We are seeking a compassionate and organized Virtual Health Concierge to join our team and support patients throughout their menopause care journey. In this role, you will be the primary point of contact for patients, guiding them through our clinical program and ensuring they receive personalized care and support.

Responsibilities

• - Conduct virtual consultations with patients to explain our care model and enroll them in the menopause clinical program
• - Guide patients through their healthcare journey, from initial consultation to follow-up appointments
• - Provide personalized assistance and support to patients, addressing their questions and concerns related to menopause symptoms and treatment
• - Coordinate with our clinical team to ensure patients are connected with appropriate healthcare providers as needed
• - Manage patient schedules, including appointment reminders and follow-ups
• - Assist patients in navigating administrative tasks, such as accessing medical records and understanding treatment plans
• - Offer education and resources on menopause, wellness, and lifestyle modifications
• - Maintain accurate and up-to-date patient records in our digital platform
• - Collaborate with the clinical team to ensure seamless patient care and communication

Qualifications

• - Registered Practical Nurse (RPN) or Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
• - Member of a professional nursing association which supplies professional liability protection.
• - Experience in patient care coordination and patient support
• - Understanding of menopause and women's health issues at midlife
• - Excellent communication and interpersonal skills, with the ability to build rapport virtually
• - Proficiency in using digital health platforms and telemedicine technologies
• - Ability to work independently and virtually with possible in person team meetings a few times per year
• - Exceptional organizational and multitasking abilities
• - Empathetic and patient-centered approach to healthcare
• - Ability to work independently and as part of a team in a fast-paced environment
