JOB DESCRIPTION

The Integrated Community Access Program is seeking a Registered Nurse 3 (Community Mental Health Nurse), reporting to the Clinical Manager of Admissions and Bed Utilization. This position will provide central intake services and outbound call centre. The successful candidate will be part of a team responsible for the coordination of Central Intake including: screening and triaging of all referrals received to Ontario Shores; communication with referral sources to ensure complete and appropriate information is received; gathering information from clients and external stakeholders; presentation of complete referrals to physician responsible for acceptance to the Centre.

The nurse will provide assessments and facilitate admission to Ontario Shores for psychiatric intervention and/or referrals for community support/intervention.

This position will also provide information and system navigation to individuals, families and other care providers.

Requirements:
•Current certificate of competence from the College of Nurses of Ontario, BScN Preferred.
•Current CPR-HCP is required.
•5 years psychiatric experience in a mental health facility or service.
•3 years experience in working in crisis nurse role.
•Sound knowledge of community resources in mental health and addiction.
•Ability to make effective referrals, reflecting in depth knowledge of community resources within the Central East LHIN and strong collaborative skills in working with community partners and health care providers
•Thorough knowledge and ability to administer mental status examinations, psychosocial and crisis assessments.
•Demonstrated knowledge of the College Standards of Practice and commitment to maintaining reflective practice standards as described by the College.
•Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required.
•Ability to work effectively individually as well as part of an interprofessional team.
•Demonstrated advanced knowledge of psychotropic medication.
•Proven ability to exercise mature, effective judgment and behaviour in emergency situations.
•The candidate will have demonstrated excellent interpersonal and effective oral/written communication skills including demonstrated excellent documentation practices.
•The ability to work with clients of diverse ethno racial and cultural backgrounds and proficiency in a second language would be considered an asset.
•Current driver's license;
•Ability to perform the physical demands of the position.
•Days, evening and weekend work is required
