Job Summary:

S R.T. MedStaff is currently seeking a Visiting Wound Care Nurse (RN) specializing in Wound, Ostomy, & Continence care for our Community Visiting Nursing and Clinic Programs. The Nurse Specialized in Wound, Ostomy, and Continence (NSWOC) will provide safe, holistic, and specialized nursing care to patients in the Woodbridge area.

Responsibilities:
  • In collaboration with the patient/family/caregiver and other health care professionals, help formulate a plan of care based on patient assessment
  • Perform detailed assessment of clients with wound, ostomy and/or continence related concerns
  • Provide nursing care to patients according to the written plan of care
  • Evaluate effectiveness of treatment plan(s) and adjust accordingly in consultation with the patient/family/caregiver and other health care professionals
  • Provide consultative services to other nurses for patients with wound, ostomy, and/or continence care needs
  • Act as a patient advocate by protecting and promoting patient rights
  • Promote patient safety and assist them in living comfortably in their home environment
  • Share nursing knowledge and expertise with other health care providers


Qualifications:
  • Registered Nurse in good standing with College of Nurses of Ontario
  • Wound, Ostomy and Continence Certified Canada - WOCC (C); Canadian Nurses Association Certification
  • One (1) year recent experience in the community
  • One (1) year recent NSWOC experience
  • Excellent problem solving, time management, and crisis resolution skills
  • Excellent interpersonal and communication skills, both verbal and written
  • Experience with Information Technology
  • Valid driver's license and reliable vehicle is a requirement
  • Second language is considered an asset
  • Experience as a NSWOC, visiting nurse, visiting registered nurse, and/or community nurse considered an asset
