The Nurse Case Manager will: respond to patient inquiries, maintain a patient caseload, utilize effective communication skills, and effective problem-solving abilities, both in a team environment as well as independently. Building strong working relationships both internally and externally are key responsibilities to meet specific team and business objectives. In addition, you will attend pre-booked training sessions, complete self- learning assignments and independent studies while adhering to SDM Specialty Health Network Standard Operating Procedures, Policies, and the College of Nurses Standards of Practice.

What you will do:
- Responsible for assigned on caseload
- Act as the primary resource & contact for patients, physicians, Health Care Professionals and vendors within the designated area.
- Coordinates & facilitates the client progress from enrollment to drug administration and throughout therapy.
- Facilitate self care through the tenets of advocacy, shared decision-making and education.
- Practice cultural competence, with awareness and respect for diversity.
- Ensure thorough documentation in patient file to ensure continuity of care is maintained and that any changes in patient care are clearly identified and transitioned accordingly.
- Provide status updates related to reimbursement; coordinate nursing requirements; blood work; pharmacy dispensing / delivery / refill requests.
- Report all Adverse Events (AE) and Product Technical Complaints (PTC) immediately or as soon as possible upon becoming aware of the event as per current policies and procedures
- Promotes communication and coordination between the MDs/pharma/patients/clinic staff and other departments within SDM Specialty Health Network
- Respect confidentiality, advocate for patients/families to facilitate positive outcomes, and ensure that ethical issues are identified and resolved according to the Canadian Nurses Association Code of Ethics
- Participate in internal and external committees.
- Plan and implement in-services and continuing education programs for both internal and external stakeholders as requested by management.
- Demonstrate commitment to Continuous Quality Improvement.
- Maintain high standards of professional practice by adhering to the ethics of the profession and keeping abreast of the latest professional advances.
- Participate in ongoing reflective practice activities.
- Assume responsibility for personal and professional development.
- Other responsibilities as required by SDM Specialty Health Network
What you will need
- Five or more years direct clinical experience
- Current professional nursing license in the province of work
- Fluently bilingual in both written and spoken languages (French & English) is an asset
- BScN preferred, however, a combination of experience and education will be considered
- Ability to provide an organized, efficient approach to case management
- Possession of excellent skills in assessment, communication, critical thinking and decision making
- Ability to demonstrate understanding of SDM Specialty Health Network policies, legal considerations and professional ethics
- Previous experience with Health Canada Guidelines for reporting adverse events is an asset
- Proficiency of computer skills including: Microsoft applications (Outlook, Word, Excel and Internet Explorer); experience with web-based applications (Salesforce)
- Ability to present to fellow colleagues
- Ability to remain objective and be a team player
- Ability to adapt to a changing environment
- Commitment to the principle of quality client-centered service delivery
- Knowledge of theory and implementation of Continuous Quality Improvement processes

Ce que vous ferez :
- Responsable de la charge de travail des patients
- Agir en tant que ressource principale et contact pour les patients, les médecins, les professionnels de la santé et les fournisseurs dans le domaine désigné.
- Coordonner et faciliter la progression du patient, de l'inscription à l'administration de la médication tout au long du traitement.
- Faciliter le développement d'une routine de soins personnels grâce aux principes de la défense des intérêts, le partage de la prise de décisions et l'éducation.
- Exercer vos compétences culturelles en faisant preuve de sensibilité et de respect envers la diversité.
- Veiller à ce que la documentation fournie dans le dossier du patient soit complète afin de garantir la continuité des soins et que tout changement dans les soins du patient soit clairement identifié et que la transition soit organisée en conséquence.
- Fournir des mises à jour des statuts ayant trait au remboursement; coordonner les exigences en matière de services infirmiers; gérer les analyses sanguines et la préparation, la livraison et les demandes de renouvellement des ordonnances avec la pharmacie.
- Signaler immédiatement ou dès que possible tous les événements indésirables (EI) et plaintes techniques sur le produit (PTC), conformément aux politiques et procédures en vigueur.
- Faciliter la communication et la coordination entre les médecins/pharmaciens/patients/le personnel de la clinique et les autres services au sein du Réseau de santé spécialisé SDM.
- Respecter la confidentialité, agir comme défenseur des intérêts des patients et de leurs familles, pour faciliter des résultats positifs, et veiller à reconnaître et résoudre les problèmes de nature éthique conformément au Code de déontologie de l'Association des infirmières et infirmiers du Canada.
- Participer à des comités internes et externes.
- Planifier et mettre en application des programmes de formation sur place et en continu, à l'intention de parties intéressées tant internes qu'externes, selon les demandes de la direction.
- Démontrer un engagement envers l'amélioration continue de la qualité.
- Maintenir des normes élevées en matière de pratique professionnelle en adhérant à l'éthique de la profession et en se tenant au courant des dernières percées sur le plan professionnel.
- Participer à des activités de pratique de réflexion en continu.
- Assumer la responsabilité du développement personnel et professionnel.
- Prendre en charge d'autres responsabilités selon les besoins du Réseau de santé spécialisé SDM.
Qualifications requises
- Une expérience clinique directe de cinq années ou plus
- Parfaitement bilingue à l'écrit et à l'oral (français et anglais).
- Être titulaire d'un droit d'exercer la pratique infirmière dans la province de travail
- Baccalauréat en sciences infirmières préféré; cependant, une combinaison d'expérience et de formation sera considérée
- Faire preuve d'un leadership reposant sur l'expérience
- Capacité à proposer une approche organisée et efficace au gestionnaire de cas
- Excellentes aptitudes en évaluation, communication, pensée critique et prise de décision
- Aptitude à comprendre les politiques, les aspects juridiques et l'éthique professionnelle du Réseau de santé spécialisé SDM
- Une expérience antérieure avec les directives de Santé Canada pour le signalement des effets indésirables
- Excellentes aptitudes en communication en anglais (bilinguisme français/anglais si nécessaire)
- Des compétences en informatique, y compris avec : Applications Microsoft (Outlook, Word, Excel et Internet Explorer); expérience avec les applications Web (Salesforce)
- Capacité à faire des présentations à ses collègues de travail
- Capacité de demeurer objectif et d'avoir un bon esprit d'équipe
- Capacité d'adaptation dans un environnement en mutation
- Faire preuve d'engagement quant au principe de prestation de services de qualité axés sur les clients
