We provide accessible, quality, culturally safe and holistic health care that supports and enhances the wellness of individuals, families, and communities. We work with our 11 First Nation community partners in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services and health promotion services. We have recently been approved as the first Indigenous led Ontario Health Team by the Ministry of Health.
The opportunity: Maamwesying North Shore Community Health Services is an Indigenous Primary Health Care Organization providing comprehensive primary care services (via the N'Mninoeyaa Aboriginal Health Access Centre and the Baawaating Family Health Team), home and community support services (designed Service Provider Organization), and mental health and addictions services within the member First Nation communities of the North Shore Tribal Council; is inviting applications for a highly skilled Nurse to join their Home and Community Support Services team. This vital position must provide comprehensive nursing care in the community setting to assist individuals to achieve optimum health and quality of life in order to remain at home independently.
This position offers flexible scheduling and work-life balance, and the opportunity to work within an extremely supportive and collaborative environment focused on quality of care. The Home Care Nurse will be expected to exercise their full scope of practice, pursue specialization, and have the ability to work independently or in cooperation with the Home and Community Care Support Services (HCCSS). Communicating and liaising with patients, family members and others within the interdisciplinary care team.
What you need to bring with you:
Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma Current College of Nurses of Ontario (CNO) registration in good standing A strong desire and commitment to making a difference in the lives of our clients. Exceptional interpersonal skills. Indigenous cultural competency training certification is an asset Experience working with the elderly in a community setting is an asset. Proven ability to apply advanced clinical nursing skills in a home care setting, i.e. wound care and palliative care is essential. Demonstrated ability to develop and facilitate health teaching. A clear vulnerable sector criminal reference check is a condition of employment. Must be able to work flexible hours including evenings and weekends based on clients' unique needs. Must have access to a reliable vehicle and a valid G Class driver's license.
