POSITION PROFILE

As a Care Manager with the Personal Support Program you will provide and promote high quality client centred care to those assigned to your caseload. Within the defined scope of practice as an RPN this includes but is not limited to completing client assessments, PSW delegations and training, evaluation and update of care plans, and client education and health promotion activities. The position requires excellent customer service skills, working in collaboration with the interdisciplinary team, and monitoring care provided to our community clients to ensure the highest level of care at all times. The role requires adherence to the principles of quality documentation and maintenance of client health records at all times.

PAY RANGE

$28.00 - $32.00 per hour (commensurate with experience)

ROLES AND RESPONSIBILITIES
  • Conducts in-home community client assessments.
  • Monitors, educates, mentors, coaches and evaluates personal support workers as defined by the RPNs scope of practice.
  • Ensures standards in documentation and practice are followed.
  • Assists in the recruitment and orientation of personal support workers (as required).
  • Participates in organizational quality improvement plans.
  • Participates in care plan development, implementation and evaluation.
  • Participates in the on call rotation.
  • Works collaboratively with the coordination team and supports as required.
  • Addresses and follows up on client and employee concerns in a timely manner.
  • Supports other AHS programs as required.


REQUIREMENTS
  • In good standing with the College of Nurses of Ontario
  • At least 1 year experience in Home and Community Care highly desirable
  • Experience working with personal support workers or in a Home and Community Care personal support program preferred
  • Experience with AlayaCare or community based electronic health records and scheduling software high desirable.
  • Current level "C" CPR certification or ability to obtain.
  • Bilingual in French and English preferred
  • Strong verbal and written communication skills.
  • Leadership skills, ability to mentor staff and facilitate program and staff development.
  • Commitment to client-centered care and evidence-based nursing practice.
  • Well-developed skills in diplomacy, problem solving and conflict resolution skills.
  • High degree of resourcefulness, flexibility and adaptability.
  • Strong knowledge of computer software including Microsoft Word, Excel and PowerPoint
  • Must have a reliable vehicle and willing to travel within the Ottawa region
