Posting Date: September 25, 2024Reference No: 024-056-PSW-MDEPosition: 0.60 FTE Part-Time Bilingual Permanent Personal Support Worker (PSW) Location: La Maison de L'Est Compensation: $22.46 to $26.88 an hour as outlined in Collective AgreementClosing Date: October 2 2024, 4:00 PMPosition Summary:A staff PSW position in our community-based Hospice residence. This position has 12 shifts in the four-week rotation regularly scheduled at la Maison de l'Est. Based on staffing needs, the successful applicant can pick up extra shifts up to full-time hours. All staff may be required to transfer to the alternate site for a shift urgency in rare circumstances. The successful applicant Key Responsibilities:Plans, coordinates, delivers and supports a team to provide day to day end-of-life palliative care for the residents in the Hospice and their families and/or significant others.
Reporting relationship:Reports to the Team Leader(s) and/or the Director of Residence Care or Nurse in Charge.
Qualifications & Background:
  • Personal Support Worker Certificate/Diploma
  • LEAP for PSW's/PACE - or willingness to obtain within 6 months
  • Current CPR
  • Physically able to assist bedridden and disabled patients
  • A strong sense of dedication to the mission, vision and values of Hospice Care Ottawa
  • Knowledge and experience in Hospice Palliative Care
  • Excellent communication skills
  • Leadership skills
  • Understands Ethics as they relate to Hospice Palliative Care
  • Ability to work as part of a team
  • Ability to give and take direction
  • Bilingualism is required
  • COVID-19: Proof of full vaccination is required and ability to continue to comply with our vaccination policy.
    Key Performance Indicators:1. Quantitative and measurable results expected2. Qualitative results to be achieved contributing to the success of the incumbent.
    • Provides guidance and direction to Residence Volunteers as necessary
    • Ensures that personal care to patients is carried out appropriately
    • Administers treatments to residents as prescribed
    • Provides emotional support to residents and families/significant others as appropriate. Refers family members to the Family Support Program as necessary
    • Maintains records through appropriate charting, ensuring confidentiality
    • Reports routinely to the Team Lead/Director of Care
    • Carries out the appropriate procedures when a patient dies
    • Carries out duties and responsibilities as designated by the Team Leads in his/her absence.
    • Participates in daily shift change meetings
    • Liaises with physicians as necessary
    • Liaises with Pharmacies as necessary
    • Mentors new staff
    • Participates in Student mentorship as required
    • Participates in education opportunities (providing and receiving)
    • Participates in annual Hospice Health and Safety Update(s) and Skills Week
    • Participates regularly in Residence team meetings
    • Participates on committees as required
    • Participates in Hospice activities (i.e. Hike, Volunteer appreciation)
