The CMHA Health Centre (CHC) is a Community Health Centre, providing a welcoming, supportive environment, free of stigma while placing the client at the centre of all care. Clients experience a coordinated, holistic approach to comprehensive primary and mental health care through education, prevention, treatment and advocacy.

The Canadian Mental Health Association (CMHA) Health Centre is a multidisciplinary team serving the population across the lifespan with specialized expertise caring for those with complex physical and mental health and addiction needs. Complex needs include chronic and complex multi-system diseases as well as serious and persistent mental health and substance use disorders. Services are provided within a team-based model using a holistic, client-centered approach to disease prevention, chronic disease management, harm reduction and health promotion.

In partnership with staff members of CMHA and partnering agencies, the Nurse Practitioner utilizes their advanced knowledge and decision-making skills to provide comprehensive health services, including assessment, diagnosis and health care management to clients experiencing chronic diseases, complex multi-system diseases, mental health and substance use disorders. The development and maintenance of such relationships is essential to the identification and execution of primary health care programs and individualized client treatment plans.

RESPONSIBILITIES & DUTIES:
  • Provides health assessment, diagnosis and critical decision making, incorporating standards of practice and organizational policies and procedures.
  • Provides clinical therapeutics in the treatment of diseases and disorders within the scope of practice of the Primary Health Care NP including pharmacological and complementary therapies.
  • Provides clinical leadership to team members through case conferencing, to address complex client health needs.
  • Collaborates with team members to implement strategies that promote health and prevent disease among individuals, families and groups


EDUCATION & SKILLS:
  • Current Certificate of Competence with College of Nurses of Ontario, Nurse Practitioner's Certificate from a recognized program, Baccalaureate in Nursing (BScN), Masters of Science Degree in Nursing (M.Sc.N.) is preferred. Registration in the Extended Class with the College of Nurses of Ontario. Current Certification in B.C.L.S.
  • Completion of Primary Health Care NP program is required.
  • Member of CNPS in good standing is required.
  • Completion of the CAN Specialty certificate in mental health is an asset.
  • Completion of the CNO approved controlled substances course/program is required, including direct experience caring for clients with chronic non-cancer pain.
  • Recent clinical nursing experience relevant to the provision of primary care.
  • Advanced clinical knowledge in relevant specialty area.
  • Knowledge in the development and implementation of policies, procedures, and educational programs.
  • Knowledge of mental health & relevant legislation preferred.
  • Ability to analyze and interpret valid research findings in clinical practice.
  • Ability to work independently and collaboratively with members of the health care team.
  • Knowledge of mental health & relevant legislation.
  • Ability to communicate effectively with clients, public, professionals and staff, in written and oral form
  • Proficiency in the use of various software applications, e.g., Microsoft Outlook, Windows and Word.
  • Bilingualism in both official languages at the advanced level is considered an asset.
  • A valid Ontario Driver's License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.

Hours of Work: Variable days/afternoons/weekends. Flexibility is required to meet the needs of clients and the community.

Salary range: $62.72 - $70.89 per hour

Per 2024-2026 Collective Agreement
