We are looking for a full-time contract Registered Nurse to join our Outreach Services team!
The Registered Nurse is responsible for assisting clients in developing skills to manage substance misuse and/or planning for solution focused, time limited services as well as provide care to clients with substance abuse/co-occurring disorders in a variety of community settings.
Status: Full-time contract until March 31, 2025. 8-hour and 12-hour shifts covering evenings, weekends and overnight. 75 hours bi-weekly.
Base Salary: $74,006.35 - $84,937.87 annually. (Candidate qualifications will be considered when determining the starting salary)
Location: This position is based out of our 60 West Drive office. Due to the nature of this program, this role is on-site and/or community based.

What You Will Do:
- To provide Nursing Care as the primary staff on the Agency's Mobile Health Vehicle.
- Build client capacity, empowerment and create self-reliance.
- Develop and initiate relationships with prospective clients based on trust, respect, and honesty.
- Maintain professional relationships with referral sources and other service providers.
- Collaborate with mental health and addiction service providers to support the mental health, addiction and primary care health needs of clients who may be in crisis or involved with community mental health and addictions agencies.
- Conduct comprehensive bio-psychosocial model of care and best practices for concurrent disorders.
- Communicate information, both verbally and in writing regarding assessments, treatment plans and evaluations to the treatment team, client, referral source and other caregivers within practice/agency/legal guidelines.
- Monitors the ongoing therapy of clients with addictions, chronic mental and physical illness by providing effective pharmacological, complementary, or counselling interventions.
- Collaborate with the Nurse Practitioner or Physician, coordinate, schedule and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment.
- Works effectively and collaboratively with other programs and services, internal and external to CMHA Peel Dufferin.

What You Need to Join Our Team:
- Current registration as a Registered Nurse with the College of Nurses of Ontario.
- Certification in psychiatric/mental health nursing CPMHN (c) is an asset.
- A minimum of 4-5 years' experience working with clients who have addictions and mental illness.
- Experience supporting clients in a community setting.
- Certification in First Aid, C.P.R., A.S.I.S.T is an asset.
- Wound care and foot care certification an asset.
- Experience with street nursing is an asset.
- Supporting Mobile Health vehicle an asset
- Valid Ontario driver's license, relevant vehicle insurance and access to a reliable vehicle.
- Current CPR- Level C & AED certification.
- A satisfactory Vulnerable Sector Screening (Police Check) report.
- Ability to work some evening shifts on a weekly basis.
