Nurse Practitioner, Primary Care Outreach to Seniors

0.6 FTE Permanent Part time (21hrs/week)

Start Date: October 2024

South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a range of multi-disciplinary services to clients, including primary care, health promotion, social services, community and resource development, home support and advocacy to address the social determinants of health. Our services and programs are designed to meet the needs of our community.

We are looking for a highly seasoned and confident nurse practitioner to join our regional Primary Care Outreach to Seniors program to work alongside our team of registered nurses to support frail seniors aging at home. The nurse practitioner will provide consultation to nurses, initiate clinical pathways and referrals, support client advocacy, and provide short-term medical management and episodic care to clients who are poorly attached or not attached to ongoing primary care. This work will primary be conducted virtually and the nurse practitioner will rely on the registered nurses doing home visits to conduct assessments, provide care, and relay pertinent information. The candidate should exhibit leadership and advocacy skills to promote and support objectives that are consistent with our Centre and the objectives of this region-wide initiative to support frail seniors aging at home.

RESPONSIBILITIES:
  • Be a resource and support to the registered nurses on the team and work in collaboration to meet client's medical needs
  • Be a strong advocate for the client and support attachment to primary care
  • Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and health education
  • Contribute to the continuous development, implementation and evaluation of the program and clinical pathways.
  • Complete timely and accurate medical records of client encounters, documentation and third-party correspondence using electronic medical record.
  • Ensure client care and caseload is managed to achieve the goals of the program


MINIMUM QUALIFICATIONS:
  • Baccalaureate in Nursing and Diploma/Master of Nursing (Nurse Practitioner)
  • Registered with the College of Nurses of Ontario - Extended class
  • Minimum seven years' experience as a nurse practitioner in a community or primary care setting providing care to frail seniors with complex medical and social needs aging at home; virtual care experience considered an asset.
  • Strong clinical skills and diagnostic abilities in complex medicine, geriatric care, psychiatry: Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and health education
  • Ability to effectively assess and manage mental health crises in a professional and culturally sensitive manner
  • Previous experience working with culturally diverse and marginalized populations
  • Strong critical thinking, problem-solving skills, organizational and interpersonal skills with an ability to work effectively within a multidisciplinary team environment
  • Ability to work with a team of registered nurses and alongside primary care providers in the community to provide transitional care to our clients
  • Knowledge of local health system and partners, a definite asset
  • Strong communication, interpersonal and conflict resolution skills
  • Proficiency in the use of computers and various software applications including electronic health records; knowledge of Telus PSS is an asset
  • Ability to thrive in a busy and fast-paced environment
  • Fluency in English; French and other languages an asset
  • Proof of Vaccination against Covid-19 required
  • Recent Police Record Check for the Vulnerable Sector required
  • Valid driver's license and access to reliable, insured vehicle (occasional travel may be required)
  • Ability to work flexible hours


Hourly Rate: $112,348 - 132,204 (pro rated to FTE, with HOOPP and benefits)
