The RPN (Nursing Rehab Specialist) will assist with the planning of and the carrying out of the activities of the nursing rehabilitation program. As a member of the interdisciplinary team, contributes to the resident's psychosocial wellbeing and improved quality of life and maintaining their autonomy and wellness.
Duties:
• Ensures nursing rehab activities in the home are implemented. E.g., walking, transfer training, wheelchair mobility, active and passive range of motion, toileting, and dining, etc.
• Assists the team in completing resident assessments for admission into the nursing rehab program.
• Proactively monitors resident progress and changes in condition to ensure prompt updating of the nursing rehab plan and activities.
• Ensures nursing rehab plan is promptly communicated to nursing staff (RPNs, PSWs) and initiates implementation.
• Participates in the interdisciplinary plan of care rounds and discusses with PT and the team who would benefit from nursing restorative.
• Closely monitors ADL scores, outcome scores, and Special Rehab RUG in relation to nursing rehab, Ensure coding accuracy and documentation consistency regarding NR care.
• Conducts restorative feeding assessments and works in collaboration with food services to ensure feeding assistive devices are in place during dining and assessed.
• Performs quarterly resident evaluations on a formal Nursing Restorative program and updates care plans accordingly.
Our ideal candidate possesses the following qualifications:
• Current RPN License with Ontario College of Nurses and Medication Administration Certificate
• Minimum three (3) year recent experience as an RPN in long-term care
• P.I.E.C.E.S. Gentle Persuasive Approach, training an asset.
• Demonstrated client centered knowledge and understanding of various mental health disorders & responsive behaviours in providing care to residents.
• Ability to work effectively as a member of a multidisciplinary team and maintain a positive working relationship with peers.
• Ability to think quickly and critically to make effective decisions assessing a comprehensive range of variables to interpret, analyze and solve problems.
• Demonstrated adaptability & flexibility to respond to emerging & dynamic program needs
• Proficient with RAI/MDS computerized documentation
• Excellent oral and written communication skills in English required.
• Positive attitude, caring and energetic personality to represent Shepherd Village in a professional and positive manner.
• Demonstrate awareness with MOHLTC Legislations specific to Safe and Secure and Residents Bill of Rights
