We are looking for a new PART-TIME PSW with experience supporting bariatric patients. Here are the high-level details:
  • PART-TIME PSW
  • 10 am to 6 pm shift
  • Our Home also has a no lift policy!


Reporting to the Resident Home Area Manager, the PSW is responsible for resident activities of daily living and supporting residents in their overall well-being. PSW's provide support with bedding, eating, toileting, transferring, dressing, grooming, mobility and cognitive and emotional support. PSW's ensure delivery of optimal care and services, based on legislative requirements, while following a resident centered approach to life at HMLTC.

Under the direction of the RN/RPN, the PSW performs assigned functions as an unregulated care provider, that support the nursing team in assisting residents with activities of daily living and supports the effective operation of resident care areas.

RESIDENT CARE & SUPPORT

Activities
  • Provides care and support to residents with all Activities of Daily Living (ADLs), including Bedding, Eating, Toileting, Transferring, Dressing, Grooming & overall well-being;
  • Supports residents with the following duties: transferring, repositioning, lifting, ambulating, weighing and other activities as outlined in their plan of care; .
  • Ensures resident rooms are conducive with a safe and support environment, including making beds and light tidying;
  • Responds efficiently to call bells and relays information to appropriate health care professional;
  • Effectively communicates to the RN/RPN, in a timely manner; resident information specific to assigned activities, observed changes in condition, and unusual occurrences, resident/family concerns, and resident behaviours;
  • Supports residents at meal and snack times; measures and records fluid & dietary intake and output and relays data nurse;
  • Operates mechanical lifts and utilizes assistive devices as directed;
  • Ensures a resident centred approach to care is followed and residents are supported in their care wishes;
  • Other related duties as assigned

  • Ensures documentation of ADL's and other findings, in a timely manner in accordance with HMLTC policy and regulations;
  • Self-actualization by helping residents to reach their maximum potential;
  • Strives to provide excellent resident centred care;
  • Build professional relationships with residents, families, co-workers and the interdisciplinary team; become a trusted member of the residents circle of care;
  • Adheres to all HMLTC Policies and Procedures, Infection Control protocols, LTCHA and Regulations, and Occupational Health and Safety policies
  • Other duties as assigned


Required Documentation
  • Original copy of 2-step TB test results OR Chest X-ray
  • Copy of 3 doses of COVID-19 vaccination proof
  • Copy of PSW Certification
  • Copy of valid CPR certificate (within 2023)
  • Copy of valid Mask Fit test (within 2023)
  • Original copy of Vulnerable Sector Screen
