Position Summary

Reporting to the Administrator and receiving professional guidance from the Medical Director, the Nurse Practitioner, Lead (NPL) collaborates with Elders and families/caregivers, along with the broader health care team in the development, implementation, and evaluation of Elders' plans of care; provides leadership and mentorship to interdisciplinary professionals in the long-term care home and leads and collaborates on education, and evidence-based practice initiatives. The NPL is the lead for the primary health care team within the long-term care home and is responsible for supervising the care provided to Elders on an on-going basis. The NPL maintains confidentiality and acts in an ethical manner, consistent with professional expectations and the long-term care home's policies and the County Code of Conduct.

Job Duties and Responsibilities

Elder Care:

•Collaborate and liaise with the MD, DON & PC, and interdisciplinary team in care delivery

•Assess, diagnose, and evaluate acute and chronic illness and other changes in Elder's conditions

•Prescribe treatments, diagnostics and follow up care

•Participate in Elder Care Conferences

•Document clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, Elder responses and clinical rationale in a timely manner

Education and Leadership:

•Support and advocate for Elder's best interests while engaging with the Elder/family/substitute decision maker (SDM) regarding care

•Provide clinical leadership to the interdisciplinary team members centered around the Elder care needs that may include move in/out, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and support for responsive behaviours

•Participate in creating an organizational environment that supports safety and quality Elder care and life, collaborative practice, and professional growth

•Participate in Quality Committee Meetings, support QA projects and initiatives

•Participate in after-hours responses and be available on-call as needed

Teaching and Coaching:

•Participate in identifying, analyzing, and interpreting trends in Elder care and professional nursing practice to determine priorities for educational programming for team members, Elders and families

•Promote knowledge development and mentorship of clinical staff by integrating best practices in Elder care

•Assist with the development and delivery of education to staff, Elders and families

General:

•Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed

•Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis

Functional Authority: As leader of an interdisciplinary team in the long-term care home, the NPL Provides guidance, direction, consultation, and advice to Registered Nurses, Registered Practical Nurses, Personal Support Workers, and other members of an interdisciplinary team. The NPL reports on activities to the Administrator of the long-term care home, as required by County policies and by professional obligations imposed by the College of Nurses Ontario.

Credentials Required

Minimum Formal Education

•A University degree from an accredited post-secondary school institution in Nursing BScN

•Current Certificate of competence from the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice. Master's Degree in Nursing is preferred

•Post-graduate certification and experience in gerontology and palliative care is an asset

•Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management

Experience

•Demonstrated knowledge of primary health issues in long-term care homes

•Excellent written and oral communication skills

•Demonstrated ability to work with an interdisciplinary team of healthcare professionals

•Willingness to engage in on-going professional development to maintain clinical competence and currency with legislation affecting practice
