A Registered Practical Nurse position is only required to work at one Home. *

Position Summary

Monitor the provision of Elder care by the nursing team on an assigned unit, acting as the team leader. Assess the Elder's needs in accordance with the RAI-MDS tool and provide a plan of care and interventions based on an evaluation of outcomes.

Duties and Responsibilities

Administration of Medications and Treatments:

• Administer and document medications and treatments for all Elders, and assess effectiveness

• Report Elder response to medication and treatments to physician/nurse practitioner

• Transcribe and process all orders received

Communication and Documentation:

• Chart care provided, behaviours and incidents

• Respond to Elder and family inquiries

• Educate Elders, families, and staff

• Complete RAI-MDS assessments, RAPs and care plans according to schedule

• Act as a mentor for students during clinical placement

• Provide guidance and direction to Personal Support Workers, and other staff working on the assigned unit

Elder Care:

• Assist as necessary with Elder care and transferring

• Ensure Elder and family are comfortable during palliative phase

• Monitor levels of liquid oxygen in portable ranks and fill as necessary

General:

• Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed

• Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis

Credentials Required

Minimum Formal Education

• Registered Practical Nurse Diploma

• Registration with the College of Nurses of Ontario
