The Nurse Practitioner, NPSTAT position is responsible for supporting initiatives designed to reduce transfers from long term care to hospital by providing direct care to long term care residents and supporting long term care homes within a designated geographical area. This is an attending position at a designated LTC home.
What will you do?
Key Responsibilities:
- Manages the delivery of coordinated quality care programs in designated long-term care facilities
- Work within the full scope of practice and autonomously diagnose, order, and interpret diagnostic tests and prescribe pharmaceuticals within the NP legislated scope of practice to avert transfers to emergency departments and hospital admissions
- Provide patient assessment, diagnosis, treatment, pain, and symptom management to clients
- Develop collaborative relationships with long term care home staff, Ontario Health atHome Care Coordinators, physicians, hospital staff and others
- Manage complex and difficult patient issues which cannot be handled in a routine manner
- Undertake collaborative consultation with physicians to maintain patient centered care
- Participate and provide advice and guidance to Ontario Health atHome staff to help manage difficult and unusual cases
- Support Ontario Health atHome initiatives and implementation into practice, including Behavioural Supports Ontario
- Support data collection and program review consistent with NPSTAT objectives
- Design and implement materials and strategies to enhance the clinical competency and capacity of long term care staff to address clinical complexity in the long term care environment
- Mentor and assist in orienting new NPs and support other clinical programs as directed by NP leadership What must you have?
Educational Qualifications
Master’s degree, in any relevant field Current RN(EC) registration with the College of Nurses of Ontario with an NP certificate in Primary Health Care or Adult Care Working knowledge of the Ontario Health atHome's role in the long term care sector A minimum of 3 years nursing RN experience A true passion for making a difference in the lives of our long-term care residents Critical thinking skills Demonstrated leadership and the ability to work autonomously and as a member of a team Collaborative Empathic and patient centered What would give you the edge?
Gerontology Nurse Certified Canada preferred (GNC) • Emergency department, mental health/dementia-related behaviours and/or geriatric experience would be an asset
• Strong clinical experience preferred
Hours of Work
Monday to Friday 10am-6pm, subject to change.
Position Location and Travel
Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
