Key Responsibilities:
• Provide holistic, client-centered care to specialized populations with barriers to care, including pediatrics, refugees, newcomers to Canada, trans care, & palliative care.
• Conduct assessments, diagnoses, planning, implementation, and evaluations in-person and virtually to your own roster of patients.
• Perform home visits for clients unable to visit the clinic.
• Collaborate with interdisciplinary teams to deliver high-quality care.
• Engage in quality improvement, research initiatives, and student mentorship.
• Utilize Electronic Medical Records (EMR) efficiently for documentation.

What We’re Looking For:
• Degree in Nursing from a recognized institution; current extended class registration with the College of Nurses of Ontario.
• Minimum of 3 years of clinical experience, preferably in a community setting.
• Passion for working with socially and medically complex patients.
• Excellent communication and interpersonal skills.
• Proficiency in EMR and virtual care tools.
• Commitment to continuous learning and professional growth.
• Ability to work flexible hours and participate in on-call schedules.
• Valid Ontario Driver’s License and access to a reliable vehicle.

Additional Requirements:
• Police Reference Check.
