Salary: Registered Practical Nurse Pay Grid ($57,000 - $60,000)
Benefits: CAAT DB+ Defined Benefit Pension Plan; Comprehensive benefit plan after 3 months.
Position Description: The Registered Practical Nurse, working in our bed-based substance use Treatment Program, performs a range of nursing and client support activities including medication administration and reconciliation, providing supportive nursing care during and between physician clinics, participating in team-based care, participating in team case conferences, working with residents to support their access to required follow up care, providing crisis supports, assessing client needs, supportive counselling, health service navigation, and other supports as required.
Knowledge, Skills and Abilities
- Assessment, evaluation, planning and revision of client care.
- Collaborate with team members and partner physicians (addiction medicine, psychiatry, family medicine) to provide effective care for residents of our bed-based substance use treatment program.
- Develop and facilitate health education groups.
- Perform delegated independent nursing functions using established procedures, policies, guidelines and standards.
- Work with supervisors to support the staff team in Infection Prevention and Control activities.
- Manage documentation and data entry regarding receipt and provision of client medications.
- Perform medication reconciliations and support team members to complete them.
- Perform tasks related to client care and the client’s environment in a safe manner.
- Support residents to access follow up to physician appointments (i.e. lab work, imaging, appointments, etc.).
- Coordinate with the pharmacy and assist with medication transfers.
- Support residents to access needed information on their medical care provided by partner physicians.
- Record client status and treatments using our client management system.
- Submit and update records of all client information correctly and in a timely fashion.
- Perform direct client care of all clients requiring skilled assessment and intervention.
- Assure that equipment used is well maintained, used appropriately, tracked and returned.
- Participate in case conferences to support effective client care.
Qualifications
- Graduate of an accredited school of nursing required, RPN License required.
- Current CPR and First Aid certification required.
- Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care.
- Ability to work in a substance use treatment context; solid understanding of addictions including alcohol and opioid use disorders.
- Experience with clients with mental health/concurrent disorders an asset.
- High degree of resourcefulness, flexibility and adaptability.
- Ability to work with a variety of client presenting situations, personalities and level of ability.
- Able to work both on a team and independently.
- Demonstration of critical thinking and prioritization skills.
- Demonstrated excellent interpersonal and communication skills, ability to work with multidisciplinary team, clients and family members
- Excellent organizational skills and able to work effectively in a fast-paced environment
- A working knowledge of client management systems/electronic health records.
- The ability to provide services in both English and French a definite asset.
