What will you do?
- HCCSS WW is committed to supporting healthcare in a manner that is consistent with patient and family centred care
- Applicants are required to have demonstrated understanding and commitment to this care philosophy
- As part of a team, Nurse Practitioners provide expert clinical care to HCCSS WW patients and expert clinical advice to primary care physicians and community nurses on the management of wounds along with other acute or chronic health conditions.
- Is the clinical resource for Care Coordinators and service providers in terms of clinical expertise in wound diagnosis, assessment and treatment.
- Provides consistent clinical support for HCCSS patients as the patients interact with home and community care, primary care, acute care, and specialist care.
- May participate in case conferences with care teams.
- Develops a shared care partnership with primary care, and wound care specialists.
- Facilitates ongoing integration of patient’s medical care across the health care sector to ensure all domains of care are addressed in a seamless, integrated manner and client and family goals are achieved.
- Ensures regular communication with direct care community team as well as primary care practitioner for each patient.
- Performing other duties as assigned.
- Current registration with the College of Nurses of Ontario
- Registered Nurse in the Extended Class (RN EC designation)/expectation to have RN EC registration and successful completion of the Nurse Practitioners’ exam set by the College of Nurses of Ontario
- Advanced wound care certification such as NSWOC, IIWCC, MCISc-WH or equivalent
- Advanced experience in wound care practice including equipment and product selection
- Familiarity with the services available within the community sector
- Demonstrated expertise in wound care and collaborating with external agencies and community partners
- Advanced assessment skills and sound knowledge of clinical therapeutics
- Demonstrated clinical leadership and collaborative practice with all care providers
- The ability to travel throughout Waterloo Region and Wellington County is required.
- Fluency in French language preferred.
- Fluency in French language preferred.
This position is a Hybrid position and will be required to be onsite in the office as determined by the Employer. The successful applicant must be willing and able to attend onsite as required.
