South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a range of multi-disciplinary services to clients, including primary care, health promotion, social services, community and resource development, home support, and advocacy to address the social determinants of health. SEOCHC particularly responds to persons who face barriers to access health and social services and who live in Southeast Ottawa. Our services and programs are designed to meet the needs of our community.
Our Primary Care Outreach to Seniors program is seeking an experienced bilingual Registered Nurse to deliver high-quality services to frail seniors in their homes. The successful candidate will work as a member of an interprofessional team working across the Champlain region. - Able to conduct comprehensive geriatric assessments to design individualized plans of care Recent clinical experience working with seniors with mental health and substance use concerns Strong skills in clinical assessment, screening, illness prevention, and disease management with a goal of improving health outcomes and facilitating access to health services Aptitude and interest in client education and health promotion with seniors Knowledge of community resources for seniors Recent experience working in a community-based setting and/or ability to work independently in the community
- Bachelor-level education in Nursing from a recognized university Registered with the College of Nurses of Ontario, entitled to practice with no restrictions Certificate of Basic Life Support (BLS) for Healthcare Provider or equivalent such as Cardio-pulmonary Resuscitation/Automated External Defibrillator (CPR/AED) Level C Valid driver’s license, insurance, and use of a personally owned vehicle Fluency in English and French Proficiency in the use of computers, software applications, and electronic documentation Vulnerable sector screening completed within the last 12 months Gerontological nursing certification and/or training is an asset
Salary range: $63,918.40 to $75,202.40 per annum (HOOPP eligible)
