What You Will Do:

ParaMed Healthcare Professionals have the clinical knowledge and specialized expertise that matches patients required level of care, to ensure their health care needs are met with every visit.

Essential Duties and Responsibilities:
Participate in scheduled visits in the community to look after patient needs within your scope of practice

Provide ethical and safe nursing care/personal support

Use appropriate assessment tools/techniques to assess patients’ individual needs

Execute and continuously monitor care plans and adjust as required

Engage patients using a collaborative approach by building rapport and trust

Maintain and update professional patient records related to patient encounters and plans

Who You Are: A Healthcare Professional – Registered Nurse, Registered Practical Nurse, Personal Support Worker, Home Support Worker and Nursing Student, that brings energy, empathy and enthusiasm to everything you do. Including supporting and encouraging every patient in every interaction.

Required Skills, Education, and Certifications:

Certificate in the field of practice

Registration with the CNO as a Registered Practical Nurse or Registered Nurse in good standing

Valid driver’s license and access to a vehicle

Reliability and flexibility

Experience working with the vulnerable population

Passion for making a difference in your community

Willingness to share in on-call coverage to ensure patients are cared for during their time of need, including after hours

Preferred Skills, Education, and Certifications:
Experience in delivering community care is an asset

Physical Requirements:

Willingness to travel to and from patient residences within a designated area
