Nurse Practitioner - Mobile Health Services Full-Time Regular, 1.0 FTE (35 hours/week), Bargaining Unit – ONA (Ontario Nurses Association)
Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a dynamic, community-based health care organization serving mid-west downtown Toronto. Offering a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming, we are also committed to serving people who face barriers to health care, and we strive to reduce these barriers. PQWCHC is a multi-site organization.
General Responsibilities:
Reporting to the Manager of Clinical and Allied Health, Parkdale site, this position is responsible for providing holistic health care to clients accessing care through PQWCHC’s Mobile Health Services. This may include providing care to individuals in buildings where they reside and in partnership locations. The NP also acts as a supervisor to health care students and participates in research and quality improvement activities.
Our clinical team works closely with our mental health, harm reduction and health promotion teams to ensure cross-team and cross-site collaboration and the NP will work with staff from those teams in providing service, as well as in program development and implementation.
A key collaborating partner for the mobile health services is University Health Network’s (UHN’s) Gattuso Centre for Social Medicine. Through applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN’s most medically and socially complex patients/clients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity.
Skills & Qualifications:
• Provision/maintenance of current and valid registration as a Registered Nurse (EC) with the College of Nurses of Ontario annually, and RNAO membership • 3 to 5 years nursing experience in a community setting, public health setting, and/or primary care • Experience with street or outreach nursing is an asset • Knowledge and experience in mental health, harm reduction and chronic illness • Strong oral and written communication skills • Excellent decision-making skills • High level of comfort with diverse populations; demonstrated ability and comfort in working with people who use drugs and people with mental health challenges • Able to work independently and as a team member within a multi-disciplinary environment. • Possess an Ontario G classification drivers license and be comfortable sharing responsibilities and driving a Mercedes Sprinter vehicle.
Compensation: $102,652 to $122,178 per annum plus group benefits and HOOPP (Healthcare of Ontario Pension Plan)
