Summary:
The successful candidate's primary focus will support early management of Chronic Kidney Disease (CKD) which includes CKD identification, prevention and the concepts of patient self care management. Responsibilities also include patient screening, preparation and promotion of independent modalities. These responsibilities will be in collaboration with the Nephrologists, the Allied Health team and the CKD clinic nurses within a mentorship environment. The candidate will contribute to ensuring there is a safe environment for patients, staff and visitors.

Other information:
- Current Certificate of Competence - College of Nurses of Ontario in good standing, BSCN preferred.
- Minimum 5 years current clinical experience in an outpatient Nephrology/CKD clinic preferred.
- Current certification in nephrology nursing
- BCLS
- Certification in blood bank administration, IV drug therapy
- Must be able to work independently and be self directed.
- Excellent critical thinking skills with experience in case management.
- Demonstrated knowledge and skill in the areas of patient adult education, Health promotion, Chronic Kidney Disease Management and Chronic Disease Management.
- Demonstrated knowledge of General Medicine/Nephrology, Peritoneal dialysis, hemodialysis, diabetes education and hypertension.
- Demonstrated effective communication skills showing the ability to work with complex patients, their families, the multidisciplinary team, CCAC and other health care facilities.
- Demonstrated experience in continuous quality improvement initiatives
- Good attendance record
- Valid Driver's License
- Evaluation method may be required as part of the selection process
