The Nurse Case Manager (CM-Nurse), handling intake calls in a professional and effective manner, conducting home assessments, nursing assessments and wellness checks with clients and their families. The role requires: diplomacy, problem solving, conflict resolution and in-depth assessment skills. The CM-Nurse has the primary responsibility of obtaining clients, conducting assessments of client's needs, interacting with families to ensure quality care and expectations are managed and delivered. The CM-Nurse must work from an empowerment model. The goal of AHH services is self-actualization of the patient and encourage the caregivers to do the same to promote optimal health and well-being. AHH patients should do as much as they are capable of doing themselves.

This position is a salary+ bonus position with daytime hours 9am-5pm Monday - Friday.

*We look for the following qualifications:*
  • You must have a valid driver license
  • Good driving record
  • Police clearance
  • Pass extensive background checks
  • Computer skills, proficiency with MS Office Suite, Google Apps, Scheduling Software
  • Experience working with seniors- 5+ years in a community nursing, supervisory, case management or administrative role.
  • RPN or RN designation
  • BSO and advocacy Experience
  • Problem Solving, Conflict resolutions, customer service skills
  • Excellent communication skills with the ability to speak, read and write in English
  • Bilingualism in any other language is a strong asset but not required
