Partners in the KW4 Ontario Health Team sub-region are launching an Integrated Care Team for Older Adults and expanding the Complex Care Program, both hosted at New Vision Family Health Team. Grand River Hospital and St. Mary's General Hospital are key partners in the success of these programs, which provide standardized assessment, treatment based on goals of care, specialist treatment, and referrals to community support services for older adults living with complex and chronic health conditions. The Integrated Care Team for Older Adults and Complex Care Program are interdisciplinary teams that work in a shared care model to support older adults to live at home for as long and as safely as possible with interventions based on clinical evidence and goals of care. Hosted at New Vision Family Health Team, these programs function as nurse practitioner (NP)-led clinics in a primary care setting, leveraging partners from: geriatric medicine, geriatric psychiatry, home and community care support services, community paramedicine, hospice and palliative care, community services, GeriMedRisk, and seniors' mental health, among others. Reporting jointly to the Program Director Mental Health & Addictions (acute adult portfolio) and Executive Director, New Vision Family Health Team, the Primary Health Care Nurse Practitioner (RN-EC) functions in an expanded role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness within the NP's area of specialty (i.e. Geriatrics). The expanded role involves collecting and interpreting data about the health of the patient, determining a medical diagnosis and treating complex responses of individuals and families to actual or potential problems in collaboration with the interdisciplinary teams of the Integrated Care Team for Older Adults and Complex Care Program. The NP performs quality improvement and administrative activities and participates in policy and procedure development relevant to system coordination and optimal patient care. Other key components of the role may include performing research activities; delivering, coordinating and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospitals/agencies.

  • Current certificate of registration with the College of Nurses of Ontario as a nurse practitioner in good standing.
  • Master of Science (Nursing) or a Masters in a relevant discipline is preferred.
  • One of Gerontological Nursing Certification or Psychiatric and Mental Health Nursing Certification preferred, or willingness to obtain certification.
  • Minimum three (3) years' recent experience in older adult health and/or seniors' mental health nursing.
  • Working knowledge and application of Gentle Persuasive Approaches (GPA) and/or Physical, Intellectual, Emotional, Capabilities, Environment, Social (PIECES) approach to care is an asset.
  • Demonstrated ability to work as a member of the interdisciplinary team and to function within a collaborative practice model.
  • Effective leadership capabilities, including but not exclusive to teaching, mentoring, and supporting shared care models.
  • Member of RNAO is an asset with additional professional liability coverage (i.e., RNAO-LAP) is required; additional relevant professional memberships are an asset.
  • Basic Cardiac Life Support (BCLS) certification required. Advanced Cardiac Life Support (ACLS) is an asset.
  • Clinical experience in primary care health settings and/or outpatient medicine settings with medically-complex older adult patients and their families.
  • Strong skills in biopsychosocial assessments, care planning, and medication management are an asset.
  • Functional knowledge of research methodology is an asset.
  • Experience/confidence in working independently, with high degree of self-direction and strong organizational skills.
  • Demonstrated excellence in interpersonal and communication skills (written and oral) including conflict resolution skills.
  • Ability to use information technology (e.g. Microsoft Office and electronic medical records).
  • Evident commitment to ongoing education.
  • Access to vehicle and valid drivers license are preferred.
