POSITION SUMMARY:

The Concurrent Disorders Program (CDP) provides care to persons aged 16 and older presenting with severe and persistent mental illness and/or active substance abuse disorder/dependency. The capacity Building Team is part of the CDP and provides expertise in consultation for patients with concurrent disorders and addictions across in-patient and out-patient programs within all medical and mental health and addictions programs. This team provides support to the interdisciplinary team in utilizing evidence based treatment, including pharmacological interventions for addictions/concurrent disorders, and demonstrates advanced clinical reasoning skills.

The Nurse Practitioner is an autonomous health care provider who works in collaboration with physicians, other members of the interdisciplinary team and clients/families to provide evidence-informed and integrated care to in-patients and registered outpatients The NP's practice is grounded in professional, ethical and legal standards using advanced nursing knowledge, skills, experience, clinical reasoning, problem solving and critical thinking. The NP builds and expands on established RN competencies with an expanded scope of practice based on legislative authority to autonomously perform the controlled acts. The NP's fundamental competencies include clinical practice, leadership, research, consultation, and collaboration. All duties and responsibilities of the NP will be consistent with the Standards of Behaviour and policies set by St. Joseph's Healthcare Hamilton. The NP will report jointly to the Director of General Psychiatry and the Director of Nursing.

QUALIFICATIONS
  • Current registration with the College of Nurses of Ontario in the RN-Extended Class with specialty certification as NP - Adult or NP-PHC required
  • Master's Degree in Nursing required
  • Proof of successful completion of controlled substances education required
  • Faculty appointment or eligibility for appointment with a School of Nursing in an academic institution
  • CNA certification in Psychiatric/ Mental Health Nursing (CPMHN(c), or commitment to complete within 5 years of hire as a condition of employment, is required
  • Demonstrates advanced knowledge of general and psychiatric nursing theory/practice, principles, interventions, medications and symptoms
  • Demonstrated in-depth expertise regarding clinical assessment and care management needs of persons living with substance use, mental health diagnoses and disadvantaged populations, including homeless
  • Demonstrated experience in shaping, leading and managing system change with multiple stakeholders
  • Demonstrated expertise in working with from a harm reduction perspective including the prescribing of pharmacological interventions (suboxone, naltrexone)
  • Minimum 5 years of demonstrated clinical experience and expertise working in mental health specialty
  • Minimum 3 years demonstrated experience and expertise working with concurrent disorders population in an advanced practice role
  • 2 years experience within a primary addiction setting preferred
  • RNAO, NPAO, CNA membership preferred
  • Current BCLS/HCP Certification required
  • Comprehensive knowledge of the Mental Health Act, Health Care Consent Act and other related legislation in Ontario required
  • Demonstrated knowledge of legislative and ethical responsibilities relating to scope of practice and healthcare required
  • Demonstrated self-directed decision making and problem solving required
  • Demonstrated excellent interpersonal skills and ability to maintain effective collegial relationships and teamwork required
  • Demonstrated knowledge sensitivity and expertise in care of seniors and their families
  • Demonstrated effective conflict management/ resolution skills required
  • Demonstrated scholarly inquiry, knowledge translation and effective research skills
  • Ability to minimize risk adhering to established policies and procedures
  • Demonstrated proficiency in computer skills (ie: PDM, Clinical Connect, MS Office/ Outlook , EPIC, GENCAN etc.) required.
  • Demonstrated data management for evaluation, interpretation and process change
  • Demonstrated good attendance record required
