Shift: 11.25 hours, continental shifts, including weekends

Summary:

Promotes collaborative team practice amongst the multidisciplinary team, promoting best practice in Complex Medical and Palliative care, including managing ventilator dependent patients, person centered care, planning of care and transition planning. Analyzes outcomes on an ongoing basis and promotes professional accountability in all disciplines. Acts as a resource for staff members, communicating appropriate knowledge, skill and judgement. Acts as a liaison with members of the health care team on the unit and other patient care areas, patients and families. Adapts to changing workloads, collaborating with colleagues re: adjusting priorities. Is responsible for planning and coordination of nursing care interventions within the multidisciplinary team. Uses planning and organizational skills to assist with bed flow and referral management. Will provide direct care for patients whose conditions are complex and outcomes unpredictable. Acts as a role model and provides consultation to all members of the health care team.

Qualifications:
  • Current Registration with the College of Nurses of Ontario
  • Baccalaureate preparation in nursing preferred
  • Must have a minimum of 5 years of general nursing experience and experience in a nursing leadership role
  • Experience in a Complexed Medical or Palliative environment preferred
  • Understanding and working knowledge of the MDS and Winrecs an asset
  • Able to lead team meetings in an effective/productive manner
  • Demonstrated excellent communication, problem-solving, organizational and critical thinking skills
  • Demonstrated reliability, dependability, and adaptability
  • Demonstrates a strong commitment to continuous quality improvement initiatives and to customer service
  • Demonstrate a commitment to continuous education
  • Demonstrated knowledge of person centered care
  • Good attendance record
  • Must be able to perform all essential duties of the position description, which includes the ability to lift and move patients, pushing, pulling and bending.
