We currently have a Permeant Full-Time opportunity for a Nurse Clinician -Geriatric Internal Consult Team in the Specialized Geriatric Services Department at our Bayview Campus.

Hours of Work:

Weekdays: Days; 8hrs; As per schedule

Summary of Duties:

As a member of Specialized Geriatric Services the Internal Consult Team Nurse Clinician provides consultative assessment and community linkage for frail seniors admitted in hospital. In collaboration with other team members, the Nurse Clinician will:
  • Administer a Comprehensive Geriatric Assessment (CGA), plan, implement and evaluate care of senior persons with complex geriatric health problems.
  • Formulate care recommendations designed to address conditions identified in the CGA and meet the goals of the senior.
  • Provide follow up to monitor implementation of recommendations and adjust plans as necessary.
  • Participates in team meetings to review client care
  • Provide education to senior patients, caregivers and coach staff on best practices

Qualifications/Skills:
  • Bachelor's degree in Nursing required. Master's degree in Nursing preferred.
  • Current College of Nurses of Ontario Certificate of Competence; current B.C.L.S.
  • Additional training and/or experience in Gerontology is required
  • Working knowledge of community resources, LHIN, RGP, and specialized geriatric programs
  • Excellent oral and written communication skills, and computer skills
  • Demonstrated ability to function on an interdisciplinary team
  • Knowledge and skills in functional assessment and the assessment of physical and mental illness in the elderly
  • Excellent organizational skills, problem solving and management skills
  • Ability to adapt to change, be flexible in an ever-changing environment
