JOB POSTING #: 913184 Position: Nurse Practitioner (NP) Site: Toronto General Hospital Department: Connected Care Program (mostly remote) Hours: 37.5 hours per week; there may be some on-call, as well as evenings and/or weekend shifts. Salary Range: $47.87 to $59.83 per hour (Commensurate with experience and consistent with the UHN compensation policy) Status: Permanent Full-time, contingent on funding University Health Network (UHN) is looking for an experienced Nurse Practitioner to support aging in place with an integrated health and social model of care. Our mission is to advance a new model of integrated health and social care in naturally occurring retirement communities (NORC), and to develop community-led solutions that provide new options for aging in place with dignity and choice. Lead partners at UHN, Open Lab and Connected Care, are working to advance this mission. This role will join the Connected Care Hub with a focus on delivering an integrated care experience for seniors and their caregivers. Our goal is to improve resident satisfaction and overall health outcomes including supporting individuals in staying at home or as close to home as possible. During the pandemic, the Connected Care Hub addressed COVID Care needs and expanded to address key gaps in delivering equitable and comprehensive care. The Hub will allow seniors to be proactively monitored and cared for working closely with primary care and home & community care where needed.

POSITION SUMMARY:

Reporting to the Senior Program Manager, NORC Innovation Centre and the Manager of the Connected Care Hub, the Nurse Practitioner will provide assessment and values-based care planning while also supporting connection to navigation and coordination of services as available in their community. The Nurse Practitioner is expected to demonstrate autonomy while assuming and having sole accountability and the authority to independently create a holistic health and social care plan, communicate a diagnosis, prescribe drugs, and order a range of diagnostic tests for NORC patients. As a nursing leader, the Nurse Practitioner is expected to provide clinical care, research, and education. Each Nurse Practitioner will be responsible for collaborative practice within a healthcare team. The Nurse Practitioner will also demonstrate leadership as a change agent and support the advancement of nursing practice within the Connected Care Hub Program by supporting, coaching, and training the program's other nursing roles. We expect the role to be hybrid work but primarily virtual at the beginning with on-site or in-home support as needed.

RESPONSIBILITIES INCLUDE:

- Providing clinical leadership to a mobile team of NORC Integrated Care Leads. - Acting as a between the NORC ICL, community providers, specialists, primary care providers, and other providers, to ensure all components of a senior's health and social care plan are available for both the patient and their care team. - Acting as a resource to home care providers and work collaboratively with an inter-professional team, family members, support network, and a variety of informal and formal service providers as needed. - Work with care team providers to provide seniors access to the right services from the right provider, at the right time - Ensuring that the patient and system-level objectives of integrated health and social care are met. - Participating in the evaluation of integrated care delivery. - Regular monitoring of Program data and metrics, synthesizing data to make recommendations or act upon trends or patterns to inform care planning. - Works within UHN's Safety and Quality policies and procedures. - Support the continued development of the Connected Care Hub.

QUALIFICATIONS:

- Completion of a recognized Master's Degree program in Nursing. - Current certificate of (licensure)/registration with the College of Nurses of Ontario as RN (EC) or in process of obtaining - Membership in the Nurses Association of Ontario (RNAO) organizations relevant to the role - Nurse Practitioner (NP) Adult or Primary Care certificate complete or pending - Current Certificate of Competence with College of Nurses of Ontario - Current certification in Basic Cardiac Life Support - Experience in primary care geriatric care preferred - Experience in general internal medicine, oncology or transplant is an asset - Experience in nursing research methodology preferred - Eligible for cross-appointment to an academic institution - Excellent verbal and written communication skills - Demonstrated experience and competence in case management is preferred, with awareness for cultural sensitivity and skills to inform and best plan and facilitate coordination of patient care. - Demonstrated recent comprehensive knowledge of community resources and health care delivery systems in acute, primary care, and community settings is required. - Excellent customer service skills, with a demonstrated ability to quickly respond to seniors and situations with resourcefulness, flexibility and adaptability is required. - Demonstrated ability to work both independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers is required. - Demonstrated leadership skills, including problem-solving, critical thinking, conflict resolution, and the ability to effectively persuade/influence the behavior of others to align with patient care objectives and desired outcomes, are required. - Excellent initiative, judgment, time management, and stress management skills are required. - Computer literacy and basic proficiency is required with a demonstrated willingness to learn and adapt to new technology. - Program planning and evaluation methodology skills an asset - Commitment to collaborative practice - Ability to perform duties in a professional and courteous manner and produce high-quality work while meeting deadlines in accordance to UHN standards - Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
