The Patient Care Coordinator is a member of the program/clinical area management team and is responsible for the overall day-to-day unit-based coordination of patient and family care, facilitation of patient flow in and out of the unit, coordination of nursing staff and resources, and support to the unit manager with attendance and performance management, as well as conflict resolution at the point of care and beyond.

What must you have?
• Registered Nurse with Bachelor of Science of Nursing (BScN) Degree or currently in the process of completing the BScN
• Current general certificate of registration and member in good standing with the College of Nurses of Ontario.
• Current BCLS.
• Minimum of 5 years of clinical experience in Inpatient Medicine required.

• Experience with QBP pathways and management of this patient population.
• Demonstrates strong clinical skills, and judgment related to the Medicine Program.
• Knowledge of College of Nurses of Ontario's (CNO) Standards of Practice and a clear understanding of the application of the Regulated Health Professional Act (RHPA) legislation.
• Ability to work independently, self-directed, and innovative to solving conflicts.
• Demonstrates commitment to professional development and learning.
• Possesses highly developed analytical, problem solving, critical thinking skills.
• Demonstrates a strong patient and customer focused philosophy in all interactions.
• Possess the ability to introduce and effectively facilitate change.
• Demonstrated ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations.
• Communicates effectively using timely, appropriate written, verbal and non-verbal means.
• Utilizes effective listening skills to maximize understanding of issues.
• Proven ability to work within a computerized environment.
• Effectively uses empathy in interactions with others.
• Proven attendance record.
• Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
• Ability to perform the essential duties of the job.
