Our Visiting nurses provide client care in the client's own home. They provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counselling to individuals and families and much more.

Job Summary:

The Registered Nurse with advanced and specialized knowledge and clinical skills in wound, ostomy and continence care who has graduated from a recognized education program is responsible for the provision and management of client and family-centered, evidence-based, efficient nursing care to individuals and families in their homes and communities. The Registered Nurse ensures care is provided within the framework of quality & risk management and the standards and scope of practice set by VON and relevant provincial regulatory bodies.

Key Responsibilities:
  • Performs a detailed holistic assessment of clients with wound, ostomy and/or continence related concerns.
  • Formulates treatment recommendations in collaboration with the interprofessional team, and develops a plan of care based on the holistic assessment of clients who require wound, ostomy, and/or continence care.
  • Provides education and mentorship experiences in order to build wound, ostomy and continence knowledge and capacity within VON.
  • Provides consultation to care providers within the organization.
  • Collaborates with the interprofessional team across the continuum of care.
  • Shares clinical knowledge relating to wound, ostomy and continence care to enable clients, families and communities to make informed choices in determining health and service needs.
  • Remains updated on current evidence related to wound, ostomy and continence best practices.
  • Provides service consistent with the standards established by VON and provincial regulatory bodies.
  • Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to standards established by VON and provincial regulatory bodies.
  • Describes and documents client and family responses to care plan interventions using established procedures.
  • Promotes the involvement of the client and family in determining health and service needs.
  • Negotiates with the client or family as necessary to ensure the effective delivery of care while respecting client choices.
  • Documents rationale for referrals, increase and decrease in visits, and for discharge based on relevant, sound data and established guidelines.
  • Refers clients to appropriate categories of nursing providers, support providers and to other disciplines utilizing established guidelines.
  • Identifies and utilizes tools and resources for relevant work and manages these resources.
  • Promotes client-directed care, self-care, and independence through teaching and health promotion.
  • Utilizes adult learning principles and established teaching materials to provide health teaching to clients and families.
  • Influences policy review and development in health care and nursing practice standards through involvement in VON, regulatory, professional organizations and community organizations.
  • Practices in accordance with the quality and risk management framework of VON Canada and Occupational Health and Safety legislation.
  • Participates in the creation of client care plans and follows up to promote continuous quality improvement.

  • Promotes a safe and healthy workplace ensuring workplace conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations and compliant with the VON Safety Management System, including all Policies, Safe Work Practices and Procedures.
  • Abides by all VON policies and work practices
  • Abides by all confidentiality and protection of personal information policies, regulations and practices and ensures appropriate safeguards are in place within their role
  • Works in collaboration with other staff in a team approach to service delivery

External and Internal Relationships:
  • Engages regularly with clients, their families and caregivers. Provides education and support to engage clients and families in independence
  • Participates actively with other healthcare professional organizations and the community to promote nursing best practice

Education, Designations and Experience:
  • Must be licensed as a Registered Nurse in the province of practice
  • Completion of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program, or
  • International Interprofessional Wound Care Course (IIWCC),or
  • Master's prepared Advanced Practice Nurse (APN) with formal education in skin and wound care.
  • Three years of experience in the community or acute care setting with a focus on wound care is preferred.
  • Experience in community and home care is preferred.

Skill Requirements:
  • Ability to observe and make effective assessment of clinical issues.
  • Ability to initiate and support health teaching.
  • Strong written and verbal communication skills that allow for effective engagement with clients, staff and the public.
  • The use of Personal Protective Equipment (PPE) may be be required
