Position summary:

The Nurse Practitioner at Runnymede Healthcare Centre will provide comprehensive care, operational, quality, and practice leadership, and promote excellence in nursing and professional practice to align with Mission, Vision, and Values of Runnymede Healthcare Centre. Utilizing advanced leadership skills, and theoretical, and clinical expertise in this specialty area, you will deliver advanced practice nursing to patients and families within medically complex and rehabilitation populations and lead organizational and program best practice initiatives and projects. You will contribute to the education and professional development of nursing/interdisciplinary staff and the professional community within these areas of specialization and participate in academic and research projects.

Responsibilities include:
Collaborate with all members of the inter-professional team to plan and deliver holistic, comprehensive, patient- and family-centred care
Perform complete, dynamic, and ongoing assessments of patients and develop treatment plans based on the best available evidence and patient needs
Use multiple data sources to make sound clinical decisions that are in the best interests of the patient
Manage complex and evolving patient care scenarios as they arise
Facilitate safe and high-quality admissions, discharges, and transfers
Engage in appropriate patient and family teaching when required to optimize health outcomes
Order and interpret diagnostic tests to guide clinical decision-making and treatment plans
Demonstrate accountability in the provision of high quality and excellent patient care
Anticipate the wide range of potential patient responses to interventions and treatment plans and intervene as required
Appropriately manage a wide range of patient responses to actual and potential health problems
Serve as a clinical resource for complex continuing care and rehabilitation nursing
Ensure integrated care plans are completed and up to date, and support nursing and inter-professional staff in delivering high quality care according to a determined care plan and assessment findings
Maintain clear, accurate, and appropriate records of all patient care delivered
Collaborate with Clinical Educators, Advanced Practice Nurses, and Professional Practice to lead the development and review of policies, procedures, protocols, and order sets in the pursuit of excellent high-quality patient care
Understand and integrate principles of resource allocation, efficiency, and cost-effectiveness in planning, delivering, and evaluating care
Lead and participate in the evaluation of models of care, including processes and delivery
Proactively identify health system gaps and collaborate internally and externally, through partnerships, to promote resolution and optimal use of health system resources
Participate in hiring, orientation, performance evaluation and termination of nursing and inter-professional staff
Supervise, guide, coach, and mentor nursing and inter-professional staff in providing high quality care
Mentor staff to manage challenging issues and patient and family needs
Identify and communicate professional practice related issues and collaboratively support staff professional development
Identify staff learning needs and collaborate with Clinical Educators in the development and delivery of education to support learning needs
Contribute to the dissemination of new knowledge and best practices throughout the organization
Act as a mentor, preceptor, and role model for nursing colleagues and other members of the inter-professional team
Collaborate with academic institutions and other education partnerships to support a high quality learning environment for staff
Identify, appraise, and apply research findings and best practice guidelines
Adopt a scholarly approach to care delivery and team functioning
Facilitate evidence-based practice by acting as knowledge broker for clinical staff and other health care providers
Collect, analyze, and use multiple sources data in the pursuit of continuous quality improvement
Lead the implementation of quality improvement initiatives and best practices in collaboration with internal and external stakeholders (e.g. BPSO)
Provide leadership and support for nursing and inter-professional staff
Promote an innovative dynamic environment recognized for clinical excellence and patient- and family-centered care
Manage and lead inter-professional rounds, patient care conferences, and other forums as required
Collaborate with Patient Care Managers and Advanced Practice Nurses to achieve hospital goals pertaining to material, fiscal, and human resource utilization
Demonstrate excellence in communication, coaching, mentoring, and team building skills
Initiate timely and appropriate consultation, referrals, and collaboration with other health care providers
Coordinate and lead complex and dynamic inter-professional and cross-departmental teams
Engage patients, families, and team members in collaborating to resolve issues in a timely manner
Exemplify best quality communication and customer excellence while working with patients, families, and the inter-professional team
Adheres to RHC's vision, mission, and values.
Follows RHC's infection prevention and control policies and procedures.
Maintains confidentiality of personal health information.
Actively participates in the Continuous Quality Improvement/Risk Management Program.
Actively promotes culturally sensitive and senior friendly patient care.
Actively promotes a culture of safety for both patients and employees by participating in safety education and training, by reporting incidents or unsafe situations to his/her supervisor, and by complying with the hospital's safe work procedures.
Represents RHC in a positive manner.

The successful candidate will have the following qualifications/experience:
Masters degree in Nursing required
Current Registration with College of Nurses of Ontario holding an Extended Class license with no restrictions
Graduate of a recognized Nurse Practitioner program
Minimum 3 years experience as a Primary Health or Adult Nurse Practitioner
Experience in Rehabilitation/Complex Continuing Care Hospital setting an asset
Proficiency in Windows Operating System, Microsoft Office Suite: Word, Excel, Access, Outlook, PowerPoint
Current Basic Cardiac Life Support Certification
(Current Advanced Cardiac Life Support Certification an asset) Demonstrated flexibility and ability to work under pressure in a fast paced environment
Excellent written and verbal communication skills
Excellent time management and organizational skills
Excellent interpersonal skills and the ability to work co-operatively and constructively within an inter-professional healthcare team
Superior facilitation and change management skills
Knowledge of project management and adult education
Research and program evaluation skills an asset
Master's Degree in Nursing, and current CNO registration holding an Extended Class license with no restrictions
