The Complex Care program comprises three units that provide care for individuals with a variety of chronic and complex medical conditions. A broad range of patient care needs are supported by expert and diverse nursing care including complex wound management, mechanical ventilation, acute and chronic palliation, and the management of tube feeds and PICC lines. An interdisciplinary team provides a holistic and collaborative approach to care focusing on the physical, psychosocial, emotional and spiritual needs of the patient and family. Our ultimate goal is to earn the complete confidence of our patients, and their families, by providing exceptional care that meets their individualized care needs. The 18 bed Palliative Care Unit, provides compassionate end of life care and pain and symptom management for individuals and their families. The Medically Complex Service comprises two units that provide care for individuals with a variety of chronic and complex medical conditions. The team provides care to patients who require ongoing medical management as well as rehabilitation following a major medical event, accident or illness. We provide support to individuals with a range of chronic complex conditions including designated beds for patients requiring chronic ventilator support. The goal of the Medically Complex Service unit is to return individuals to the community; either to their home or to a long term care setting. This position will provide coverage to all units within the Complex Care program. Essential Qualifications

Graduate of a recognized Personal Support Worker Certificate Program from an accredited education facility
Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Successful completion of the Fundamentals of Palliative Care course or ability to successfully complete within 12 months
Registered to attend the APPS (Advanced Palliative Practice Skills Program) course or ability to successfully complete within 24 months
Experience and knowledge of the care needs of continuing care patients
Capable of establishing positive interpersonal relationships
Ability to work in the team environment
Demonstrated good oral and written communication skills
Ability to apply effective body mechanics and meet the physical demands of the position which includes heavy lifting, bending, standing, walking, pushing, pulling
Demonstrated flexibility and organizational skills

Preferred Qualifications

Ontario Secondary School Diploma from a recognized school in the Province of Ontario
Proficiency in French would be an asset

Immunization Requirements

Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph's Health Care London policies and processes related to Covid management in the workplace.
Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
Provide documentation of the Tuberculosis skin testing
