The Hereditary Kidney Disease clinic is a provincial resource designed to improve the healthcare of patients with these rare and complex kidney conditions. The Nursing Clinical Coordinator in the Hereditary Kidney Disease Clinic, in collaboration with the healthcare team, will facilitate patient care, improve the education of the patient and referring physicians in these kidney conditions with the ultimate goal of preventing kidney disease progression. As a member of the Ambulatory Clinic (HKD) team, the responsibilities of the Clinical Coordinator, include: utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities, the coordination of patient care across the continuum and continuity in patient care management; performing patient care responsibilities; performing teaching/educational responsibilities; participating actively in Quality Improvement initiatives; participating in academic and research activities/projects; performing administrative responsibilities; performing cross-functional and/or other duties consistent with the job classification, as assigned or requested.
QUALIFICATIONS

Bachelors of Science Degree in Nursing
Completion of a recognized Masters degree nursing program preferred
Certificate of competency (licensure) from the College of Nurses of Ontario
Completion of Basic Cardiac Life Support (B.C.L.S.) program, Level II Rescuer certification
Up to three (3) years of relevant experience including experience working in an advanced Clinical practice role
Three (3) to Five (5) years experience in Nephrology nursing required
Effective organizational, interpersonal and communication skills
Research experience preferred
Excellent decision-making and problem solving skills
Excellent time management skills
Excellent teaching skills with the ability to teach one-on-one and/or groups
Ability to function collaboratively within a multi-disciplinary team and/or independently
Ability to work in a self-directed manner
Demonstrated awareness of and adherence to professional and ethical standards
Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
Computer literacy
Membership with the RNAO preferred
Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
