COMPENSATION:$90,899 - $120,599 Annually Prorated

To help fulfill its mission, LOFT is hiring a Nurse Practitioner.

JOB SUMMARY:

Working as a clinical leader and collaborative member of the inter-professional team, the Nurse Practitioner (NP) functions in an expanded role demonstrating a high level of autonomy and expert skill to formulate clinical decisions for individuals experiencing psychogeriatric issues and wellness promotion for seniors living in the community. The NP provides direct patient care focusing on treatment planning, health promotion, maximizing patient safety and function to support seniors living at home. The NP will be responsible for working with individuals from diverse backgrounds, contributing to Integrated Psychogeriatric Outreach Program development, participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment. The NP is directly responsible for developing and articulating strategies for the management of recovery and psychosocial rehabilitation using information collected through assessments and collaboration with the integrated care team. Additionally, this role will liaise with community agencies, other practitioners, and key partners, including care partners, to coordinate care for clients.

WHAT YOU WILL DO:

  • Conduct comprehensive psychogeriatric assessments of the older adults referred to TPH
  • Support in review and triage all referrals;
  • Provide clinical support to other members of The Path Home Team
  • System/resource navigation and case management coordinating care for patients as needed;
  • Liaise, collaborate and coordinate with community/healthcare providers, primary care, pharmacists, and partners within The Path Home Program, support implementation of clinical plan;
  • Develop short-term therapeutic relationships to foster coping and adaptation to health promoting strategies or chronic conditions;
  • Identify changes in appearance/behaviours, reporting physical and mental health concerns, unusual or exceptional occurrences and behaviour;
  • Provide support with behaviours arising from cognitive issues, and contacting appropriate emergency services in crisis situations;
  • Monitor client's symptoms and initiate interventions to facilitate stabilization when required;
  • Maintain all records of service and ensuring supplies and equipment are readily available;
  • Conduct mandatory assessments and provide timely and accurate record keeping and reports as required;
  • Assists with application of behaviour management strategies under the guidance of the geriatric psychiatrist working with The Path Home Team
  • Develop respectful and trusting relationships with clients using recovery and psychosocial rehabilitation principles;
  • Maintain strict confidentiality and follow all other required policies and procedures;
  • Effectively participate as a member of the LOFT Team; attend staff/team meetings, client care conferences, other meetings etc. as required;
  • Follows standard infection control practices and enhanced precautions as instructed;
  • Promotes client and family focused care with a culturally diverse environment;
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on LOFT Community Services;
  • Attend and actively participate in all required training;
  • Collect data as required for statistical reporting purposes;
  • Participate in team building, program development and quality improvement activities;
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations;
  • Making referrals to appropriate services;
  • Provide ongoing client education;
  • Other related duties as required;


WHAT YOU WILL BRING:

  • Registered with the College of Nurses of Ontario as an Extended Class RN and is a member in good standing;
  • Minimum 5 years of experience working with vulnerable older adults and seniors with serious mental illness, cognitive, addiction and responsive behavioural challenges;
  • Membership in the Nurse Practitioner Association of Ontario preferred;
  • National certification in Gerontology Nursing from the Canadian Nurses Association preferred;
  • Advanced clinical knowledge of current best practices in psychogeriatric assessment and treatment planning required;
  • Excellent assessment skills, including demonstrated knowledge of crisis intervention skills;
  • Experience in Case Management and Care Coordination preferred;
  • Knowledge of social services and community resources and supports for older adults;
  • Current BCLS/CPR certification required;
  • An understanding of recovery and psychosocial rehabilitation principles and the therapeutic relationship;
  • Knowledge and experience working with relevant legislation (i.e. SDM Act, POA, Consent and Capacity, etc.);
  • Advanced nursing assessment skills and sound knowledge of clinical interventions for geriatric population;
  • High level of initiative and self-direction;
  • Exemplary interpersonal and communications skills;
  • Self-directed, results oriented and capable of working independently as well as in a team environment;
  • Excellent oral and written communication skills and computer literacy;
  • Well-developed problem-solving abilities and interpersonal skills;
  • Excellent assessment skills, including demonstrated knowledge of crisis intervention skills;
  • Demonstrated ability to provide individual/group health teaching for residents and staff;
  • Ability to collaborate effectively with other members of inter-professional care teams, clients, family members and community team members;
  • Demonstrates awareness and understanding in working with individuals from different cultures;
  • Demonstrates initiative and creativity in the workplace;
  • A positive attendance record;
  • Criminal Records Check and Vulnerable Sector Screening;
  • Valid Driver's License and Vehicle preferred but not required
