POSITION: HURON PERTH ASSERTIVE COMMUNITY TREATMENT TEAM

POSITION SUMMARY The Huron Perth Assertive Community Treatment Team staff will endeavor to reflect the community it serves by providing assertive community treatment and outreach to residents of Huron and Perth counties with long term serious mental health concerns. Treatment provided will be based on clinical needs as determined mutually by clinician, client and family/friend. Mobile, flexible treatment will be provided to support those clients, whose intensity and frequency of support cannot be met by existing resources. QUALIFICATIONS Education Required - Registered Nurse and current registration with CNO - Certified in Psychiatric and Mental Health Nursing (CPMHN(C)) through the Canadian Nurses Association - Extensive knowledge of the Recovery Principles and Recovery Model - Completion of Non Violent Crisis Intervention Training - Knowledge of the Mental Health Act within Ontario Preferred - Certification of Dialectic Behavioral Therapy and Cognitive Behavioral Therapy - Certification in Solutions Focused Therapy - Completion of Suicide Training Program - Completion of a Crisis Intervention Certificate Program - Current CPR Certification and First Aid Experience - Minimum three years current progressive clinical mental health and addictions in a health care setting and community based setting along with a strong desire to work in the community to support these individuals - Minimum three years experience in responding to crisis situations with a variety of individuals in a healthcare and/or community based setting Abilities - Mature, flexible, and self-directed. - Well-developed time management and organized skills. - Able to function cooperatively within Huron and Perth multidisciplinary teams. - Well-developed verbal and written communication skills. - Able to support Huron and Perth clients in their homes or in a community environment. - Well-developed skills in assessment, case/team management, crisis intervention, group counseling/therapy, treatment planning and service coordination for adult mental health clients. - Able to facilitate the reintegration of clients currently in Tertiary/Specialized Hospital beds back into their community. - Able to support the consumer being at the center of the mental health system. - An understanding and appreciation of rural communities. - Able to work cooperatively with community and professional groups. - Able to perform responsibilities with a minimum of supervision. - Knowledge of issues faced by individuals in acute mental health distress and by individuals with severe mental illness. - Knowledge of community services and mental health treatment and support programs within Huron and Perth Counties. - Knowledge of mental health reform. - Ability to conduct formal mental health assessments. - Ability to identify and mobilize resources in crisis situations. - Personal transportation and hold a valid Ontario driver's license. - Obtain and maintain auto endorsement insurance for the purpose of client transportation during work. - Computer literate. The HPHA values that the Patient/Customer experience is as important as the outcomes. POSITION SPECIFIC: Clinical - Delivers 75% or more of the services out of the programs offices. Provides comprehensive assessment, assertive treatment, and consultation of HPACTT clients. - Works in collaboration with other HPACTT members, through a shared responsibility and governance model, in providing discipline specific. - Works in collaboration with community service providers within confidentiality guidelines. - Works in collaboration with district supports in addressing specialized addiction, vocation and dual diagnosis related issues. - Completes tasks specific to and within staff's professional designation, as identified by licensing body. - To work in collaboration with all HPACTT team members in providing support services to ensure clients obtain basic necessities of daily life. - Emphasizes outreach, relationship building, individualization of services and client choice. Case/Team Management - Maintains caseload as assigned by Coordinator of Huron Perth Assertive Community Treatment Team. - Works within the HPACTT philosophy in supporting individual clients to assess their needs and develop a personal plan. - Assists the client in developing and accessing community services and supports in response to needs (e.g. leisure/recreation, housing, medical needs, vocation). Education - Provides resource information and presentations to professional and community groups on topics related to the Huron Perth Assertive Community Treatment Team model and mental health. Community Development - Participates on community boards and committees related to adult mental health issues. - Identifies program and service gaps related to mental health needs in the Huron Perth district. - Advocates on behalf of individuals with serious mental health concerns within the Huron Perth district. Administrative - Maintains current and accurate case records and statistics of client involvement per program standards. - Participated in team's ongoing review and development of HPACTT goals and objectives. - Participates in HPACTT quality assurance/risk management, continuous quality improvement and accreditation activities. Professional - Identifies and pursues professional development in accordance with professional practice standards. - Maintains membership in appropriate professional organizations and interest groups. - Works within the boundaries by respective college/association. Other - Staff will be expected to obtain and maintain auto endorsement insurance, for the purpose of client transportation during work. - Assignment on a team rotation schedule, which provides 7 days/week service.
