*Each ComForCare office is independently owned and operated." POSITION SUMMARYProvide supplemental staffing support to 2 LTC homes in downtown Toronto. The Registered Practical Nurse provides nursing services utilizing a comprehensive base of nursing theory and process, following the College of Nurses of Ontario , Agency and Facility Policies and Procedures. REPORTS TO: Managing Director / Facility shift manager QUALIFICATIONS
  • Graduate of an accredited school of professional nursing.
  • Current license as a Registered Practical Nurse in the province of Ontario.
  • Minimum of one (1) year experience in an acute, long-term care setting or equivalent experience.
  • A driver's License is not required
  • Current CPR certification.
  • Vulnerable Screening Sector Police check clearance (Valid within one year)
  • Negative TB test within one year
  • Proof of Covid-19 vaccination receipts (Subject to any accommodation required by applicable human rights legislation, it will be a condition of employment that all new hires have received three doses of the Covid-19 vaccination within 14 days of the start date to satisfy the current Long Term Care Home requirements)
  • Experience with Point of Care-(POC) PointClickCare charting
  • Strong communication, interpersonal, and critical thinking skills
Responsibilities:
  • Provide nursing care to patients following current care plans.
  • Evaluate the effectiveness of treatment plan(s) and adjust accordingly with the appropriate care team members.
  • Report/communicate observations to the appropriate individual(s) as necessary
  • Maintain confidentiality of patient and facility information
  • Effectively communicate care plans and client information with all team members and associated service providers.

Schedule:
  • 8 and 12-hour shifts
  • Must be available to work a minimum of 4 shifts per month
  • Must be available to work every other weekend
