Reporting to a Manager, Patient Care, the Nurse Practitioner (NP) is primarily responsible for providing leadership and clinical care to support Seniors and complex clients to 'Age in Place' within HCCSS HNHB. The NP will liaise with primary and secondary care providers and targeted hospitals to facilitate hospital discharge, and prevent ED visits when possible. The NP in his/her leadership role and as a member of the team, will function at full scope of practice to plan and implement a clinical service program to assist in meeting clients' primary care needs and in meeting project goals.

Primary location will be the Niagara branch; however, you will also be required to support the Burlington branch from time to time.

Duties will include, but not be limited to, the following:
  • Provides comprehensive primary health care services to patients encompassing health promotion, prevention of diseases and cure, rehabilitation, and support services, as per the RN (EC) scope of practice guidelines
  • Plans, develops and delivers programs and/or services related to the Targeted ALC Strategies project
  • Initiates, benchmarks, evaluates, recommends, and implements best practices in the delivery of services
  • Supports an organizational culture that fosters excellence in the provision of care and multi-disciplinary teamwork
  • Collaborates with and provides expertise and guidance to case managers
  • Develops, implements and monitors for improvement policies and procedures
  • Participates in continuing education and training initiatives at HCCSS, including mentoring team members
  • Interacts with leaders to promote consistency and best practices
  • Participates in performance measurement and utilization management
  • Promotes communication to develop a collaborative multi-disciplinary care team
  • Promotes a continuous learning environment
  • Anticipates changing priorities and recommends reallocation of resources as required to meet overall needs
  • Monitors, tracks and prepares reports about the Targeted ALC Strategies project for the Director, Patient Care, as requested
  • Leads and/or participates in and demonstrates an understanding of quality, risk and client safety principles and practices
  • Follows all safe work practices and procedures and immediately communicates any activity or action which may constitute a risk to quality, and client safety
  • Promotes patient safety in alignment with the Vision, Mission, Values and Strategic Directions of HCCSS
  • Works within the basic principles of patient safety by doing the right thing for the right patient, using the right method at the right time
  • Adheres to HCCSS patient safety policies and procedures

  • Certificate of Registration in the Extended Class from the College of Nurses of Ontario, and eligible to practice as a RN-EC
  • Preferred 3 to 5 years experience in home or community care environment. Professional experience in health care should include: linkages with community health agencies and other health disciplines, needs assessment, and case management and/or discharge planning
  • Current CPR Certification

  • Knowledge of HCCSSmission, vision, and strategic directions
  • Knowledge of best practices related to service delivery
  • Significant understanding of HCCSS relationship with HCSS and broader HNHB client service community
  • Ability to research, analyze, and evaluate best practices program development and implementation and in service delivery
  • Proven interpersonal, team, and communication skills demonstrating a client service orientation
  • Establishes and maintains collaborative and effective working relationships with HCCSS employees, physicians, and with peers throughout HCCSS network
  • Ability to deliver information effectively, verbally and in writing, in a variety of settings
  • Good coaching and facilitation skills
  • Provides leadership in meeting clients' primary care needs and achieving pilot project goals
  • Computer skills in a Microsoft Office environment
  • Fluency in English (oral and written)

  • Works in the HCCSS HNHB geographic areas to support targeted hospitals
  • Regular travel within designated area of HCCSS HNHB to meet with clients and partners and provide consultation to case managers
