Job Description
Summary:
The Licensed/Registered Practical Nurse (LPN/RPN), under the direction of the immediate Supervisor, establishes, monitors and delivers nursing care to the clients/patients and their families through client/patient centered, goal oriented care plans and in accordance with all required internal and external professional practice standards. As required, LPN/RPN provides support and direction to paraprofessional staff involved in direct client care.

Education
Graduation from an accredited Licensed/Registered Practical Nursing Program: current registration with the provincial licensing body. The position also requires a solid knowledge of the principles, practices, and methods of community nursing. Clinical certificate and post grad courses would be an asset.
Experience
At least two years of recent experience; demonstrated clinical comptencies; new grads will be considered for hiring on an invidual basis, providing they meet all other hiring criteria and that a mentorship program is implemented and the new grad is closely supervised.

As required, LPN/RPN provides support and direction to paraprofessional staff involved in direct client care.
Duties and Responsibilities:
- Provide client/patient care by utilizing the skills of observation, assessment, nursing diagnosis, counselling and health teaching.
- Provide care according to the established care plan ensuring that, on an ongoing basis, the emotional, spiritual, physical comfort and safety of clients/patients are met to the greatest possible extent; care must be within the scope of practice of a Licensed/Registered Practical Nurse.
- Perform routine nursing duties in accordance with company policies and procedures and standards of Nursing practice as defined by the provincial licensing body.
- Perform specialized nursing procedures for which recognized training and current competencies can be demonstrated
- Observe and document client conditions and changes following proper standards and in accordance with professional and Healthcare Standards.
- Receive and transcribe physician's orders in accordance with provincial regulations/standards.
- Provide direction, support and delegation to paraprofessional employees (PSWs, HCAs, CHWs, etc.) as appropriate and in accordance with provincial regulations/standards.
Safety Responsibilities
- Comply with Health Care and Safety policies and procedures, safe work practices and requirements, including but not limited to Infection Prevention and Control.
- Take reasonable care to work safely and carry out duties as to not cause accidental injury to self, fellow employees or the general public.
- Compliant with health and safety policies, procedures and standards.
- Participates in proactive Health & Safety activities while performing all duties.
- Immediately report all injuries, near misses or protentional hazards to direct supervisor or alternate as appropriate.
- Always maintain and promote a safe working environment.
- Maintain awareness of the role and participate in emergency response and/or drills as required.
- Review and maintain all workplace harassment and/or violence policies and expectations.
