POSITION SUMMARY:

The Personal Support Worker is responsible for providing assistance with all activities of daily living, as well will support and follow the plan of care for behaviour management and continence care. The PSW/HCA will promote independence and foster an environment that is safe and home-like. The PSW/HCA works in accordance with all the Policies and Procedures of the Home. While performing any and all duties of the position the employee will work in a safe and healthy manner and will follow the regulations outlined in the Occupational Health and Safety Act outlined in the organization Health & Safety Policy, Infection Control and safe work procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

RESPONSIBILITIES:
  • Under the direction of the Registered Nurse and/or the Registered Practical Nurse assumes the responsibility for providing direct care to the resident according to the care plan in the following areas: Activities of Daily Living Bathing:
    • Assist or provide hygiene through a bed bath, sink bath, tub bath or shower offered at least twice weekly as per the resident choice.
    • Assist with oral hygiene by cleaning dentures, assisting with brushing teeth or mouth care for the ill or palliative care resident.
    • Assist with or provide hair washing twice weekly or as required.
    • Daily combing or brushing of hair.
    • Provide peri care in accordance with incontinence management or as required by resident.
    • Daily and as required hygiene and washing.
    • Assist with or provide fingernail or toenail care for non-diabetic residents.
    • Clean outer ear only.
    • As directed by RN or RPN may administer foot soaks with additives added by RN or RPN
    • May use medicated shampoo and bath soaps as ordered and directed by registered staff.
    • May apply over the counter (OTC) topical creams and ointments according to John Noble Home policy for applying creams, ointments.

JOB DESCRIPTION

Dressing:
  • Assist with or dress the resident daily each a.m. according to resident's choice and appropriateness of occasion, comfort and season.
  • Change soiled clothing.
  • Assist with bed time care of resident.
  • Assist resident in the care and storage of their clothing and the accessing of laundry service.
  • May apply support stockings.

Toileting:
  • Provide a safe and direct route to the resident's bathroom.
  • Provide commode, bedpan, urinal as requested and appropriate and in a timely manner.
  • Monitor intake and output for amount and characteristics and report and record as requested by RN or RPN.
  • Collect urine specimen's routine and midstream as requested by RN/RPN.
  • Collect stool specimens as requested by RN or RPN.
  • Provide colostomy care.
  • Empty catheter drainage bags and record amount as requested by RN/RPN.
  • Change regular drainage bag.

Transferring:
  • Assist resident requiring help to move from one piece of equipment or furniture to another according to the determined method identified for that resident. Transferring directions are noted in the care plan.
  • Mechanical lifts must be used when identified for use according to John Noble Home Minimal Lift policy.

Feeding:
  • Assist in the delivery, set up and feeding of residents as required.
  • Assist residents to dining room as required.
  • Monitor resident for difficulties chewing and swallowing and report to RN and /or RPN. Follow specified instructions by Speech Language Pathologist and Resident Care Plan.
  • Encourage and assist resident in maintaining good eating habits by recognizing likes, dislikes and refusal of food and report to RN/RPN.
  • Ensure dentures are used and in good repair.
  • Record daily food, and fluid consumption.
  • Provide oral care after each meal.
  • Push fluids in hot weather, when cold symptoms are present or when requested by RN/RPN.
  • Provide mid-meal snacks.
  • Provide supplemental nourishment as directed by RN/RPN.

Additional Duties:
  • Ensure knowledge of Care Plan/Kardex.
  • Maintain a tidy and safe workplace.
  • Take temperatures using Thermoscan as requested by RN/RPN.
  • Take sputum specimens as requested and per John Noble Home policy on sputum collection.
  • Take resident weights monthly and record on POC.
  • Assist with ambulation and range of motion or other passive exercises as requested.
  • Apply splints as directed.
  • Documentation - POC and other records.
  • Give input into care plans.
  • Provide care of body after death.
  • Provide emotional support to resident and SDM/Family as appropriate.
  • Apply and monitor restraints according to John Noble Home least restraint policy and as directed by RN/RPN.
  • Attend Interdisciplinary Conferences as requested by RN/RPN.
  • Attend home area meetings and other meetings as required.
  • Assist with the writing of policies and procedures as requested.
  • Completes all mandatory education and in-services according to John Noble Home Policies and Procedures.
  • Other duties as assigned.
