The Assisted Living Department provides residents with physical and cognitive limitations with a safe and secure living environment catering to their unique needs. The hallmark of Chartwell's Assisted Living Program is to provide highly individualized care that allows residents to experience the exceptional service provided in Chartwell Retirement Residences.

The Assisted Living Manager is a professional nurse who partners with residents, their family members and all members of the Assisted Living Team to deliver programs and services that ensure residents enjoy a lifestyle and quality of life that exceeds their expectations. This role is "people intensive" with a substantial component of the job involving residents, their family members and team interaction.

The Assisted Living Manager will work collaboratively with the Health & Wellness Manager in planning, leading and managing all aspects of the Assisted Living Department including directing the Assisted Living team members and executing the Assisted Living Program according to Chartwell policies and procedures.

The responsibilities include but are not limited to:
  • Ensures residents receive the appropriate level of care to meet their needs.
  • Acts as the contact person for the resident and their family members and responds to concerns.
  • Completes required paperwork for resident admissions and discharges, as well as, on-going documentation.
  • Effectively uses the nursing process of assessing, planning, implementing and evaluating to provide high quality and highly individualized care to the residents.
  • Ensures the resident and/or their family members are involved (where appropriate) in decisions affecting their care.
  • Establishes priorities based on individual resident's needs, scheduled activities and adjusts these priorities and activities to manage unanticipated events.
  • Coordinates and leads resident meetings, family care conferences and other related meetings.
  • Participates as a hands-on member of the work team and provides support to the other members of the team.
  • Participates in hiring, termination and training of staff who work directly with residents.
  • Works with, monitors and supervises the members of the care team.
  • Consults with and delegates appropriate activities to the Guest Attendants and Nurses ensuring adequate levels of resident care and supervision.
  • Monitors the activities of Nurses and Guest Attendants with respect to the direct care provided to residents.
  • Supports the onboarding and initial training of Guest Attendants and Nurses.
  • Provides on going education and training to team members, including a focus on the "caring with you" philosophy.
  • Participates in scheduling and assignment of team members.
  • Communicates with family members, physicians and outside community health care agencies.
  • Participates in community outreach.
  • Develops trusted partnerships with external care providers and care influencers.
  • Adheres to current legislative requirements, as well as, the approved policies and procedures of Chartwell Retirement Residences with respect to nursing care, and as the department manager, ensures all team members are also adhering.
  • Understands and follows all health and safety policies and procedures. Works safely to reduce the risk of injury to self, other staff members and residents. Promptly reports all actual or potentially hazardous situations.
  • Maintains confidentiality of resident's personal information.
  • Ensures adherence to operational budget, including monitoring of labour and overtime.
  • Collaborates with the Health & Wellness Manager in the purchase of supplies and equipment.
  • Assumes responsibility for the safe administration and storage of medication. Delegates responsibility where appropriate.
  • Ensures the integrity, accuracy and timely completion of resident records, maintaining resident privacy.
  • Ensures a safe environment for the resident and/or their family members.
  • Refers, reports and records pertinent information related to the resident's care to other care team members.
  • Co-ordinates and/or carries out the action plan related to the prescribed care plan.
  • Collaborates with the Health & Wellness Manager to ensure bi-annual assessments are completed and in compliance with Chartwell's Assisted Living model and regulatory standards.
  • Communicates effectively with physicians, pharmacy and other professional care providers.
  • Collaborates with other members of the care team in the planning, provision and evaluation of services.
  • Provides required in-service training and orientation to new employees, under the guidance of the General Manager.
  • Promotes collaboration with members of the multi-disciplinary team, including the Lifestyles & Programs Manager and Food Services Manager to ensure all programs and resources are provided to residents.
  • Coordinates services to ensure all Assisted Living areas are maintained in a clean and safe manner.
  • Assumes responsibility for their own actions.
  • Assumes responsibility for maintaining/upgrading their competencies related to clinical practice and professional responsibility.
  • Attends management meetings and participates in overall management of the residence
  • Participates in Chartwell's Performance Review program/process.
  • Performs other related duties as required.

The ideal candidates will possess:
  • A minimum of two years relevant experience in a healthcare setting.
  • Registered Nurse or Registered Practical Nurse. Must possess a current certificate of competence with the College of Nurses of Ontario.
  • Experience working with geriatric population, including individuals with dementia, Alzheimer's.
  • Knowledge of the Retirement Homes Act, Nursing Act and other relevant legislation.
  • Previous management experience preferred.
  • A self-directed, accountable individual with experience in progressively responsible roles with proven leadership and interpersonal skills.
  • Ability to lead and influence a multidisciplinary team.
  • Ability to provide training and ongoing education.
  • Strong assessment skills.
  • Strong verbal and written communication skills with the ability to effectively communicate with team members, residents, family members and external care providers.
  • Knowledgeable about the nursing process in relation to the geriatric care field.
  • May require working multiple shifts including weekends, evenings, nights and holidays.
  • May require being "on call".
  • Ability to respond to emergency situations (as per Chartwell policies and procedures).
  • Current CPR and First Aide Certificate.
  • Valid provincial driver's license required and access to reliable vehicle.
