We are currently looking to hire a:

Advance Practice Nurse – High Priority Community Strategies   

 

1 FTE (35 hours /week) – 6 month Contract

About the Position

The High Priority Community Strategies is moving towards sustainability and recovery. Seeking to implement positive change in the Scarborough Community. We are addressing post pandemic needs including chronic disease management. This role will work independently and with a diverse team to develop, implement, and evaluated initiative to support the social determinants of health.

 

Reporting to:  Interim Manager of Special projects

Key Areas of Responsibilities

 

 

  • Promote evidence-based clinical best practices including treatment, scanning, and chronic disease management and public health/immunization Develop and support campaigns, and individual or group health education sessions on chronic disease management and health behaviors.  Build individual/community capacity in the social determinants of health. Integrates knowledge from public health sciences, primary health care, nursing science, and the social sciences to promote, protect and preserve the social determinants of health. Use health data to inform the planning and provision of health promotion services to the community. 

Educational and/or Professional Qualifications

 

  • Master's degree in nursing from an accredited University Registration and in good standing with the College of Nurses of Ontario Minimum three (3) years experience in direct client care Clinical experience within a community-based healthcare setting preferred Member of RNAO A certificate/Diploma in Community Public Health is considered an asset Demonstrated experience/confidence in working independently, with a high degree of self-direction and strong organizational skills Oral and/or written fluency in other languages relevant to providing service in the role would be considered an asset

Experience

 

  • 2-3 in public health working in diverse communities Program development and implementation or evaluation are considered an asset Experience facilitation and health promotion  

Significant Working Conditions

 

  • Flexibility of hours – occasional evening or weekend work may be required Flexible working sites A valid driver’s license required Vehicle required

Remuneration: $38.42 - $44.82

Pay Band: B
