The Multi Organ Transplant Unit (MOTU) is Canada's largest dedicated transplant ward, performing over 450 organ transplants annually. The program currently offers the following types of organ transplants: heart, lung, liver, living donor liver, pancreas, pancreas/kidney, kidney, living donor kidney, and small bowel. The transplant patient population includes new organ transplant recipients, living organ donors, pre-transplant liver patients, as well as transplant recipients readmitted with various transplant related health issues. In addition, the unit provides urgent surgical work-up for listed patients called in for immediate organ transplant surgery. The unit provides a challenging practice setting for health care professionals. As the largest group in the team, nurses can develop broad scope of practice within this distinct nursing specialty. A strong team of pharmacists support the complex medication regimen required by transplant recipients. Dedicated physiotherapists and occupational therapists are aligned with the program. Other valuable team members include patient care assistants and ward clerks. The medical team encompasses staff physicians and surgeons who specialize in the management of transplant recipients, nurse practitioners and highly qualified fellows. This 62-bed unit is built on a strong foundation of adaptability, interdisciplinary teamwork and ongoing learning in an environment that encompasses both medical and surgical care. As a member of the MOTU Nursing and multi-disciplinary team, the REGISTERED PRACTICAL NURSE practices in accordance with relevant legislation/standards, the UHN Professional Practice Model and the mission and vision of nursing at the UHN to provide compassionate, collaborative and competent care, while meeting the legal and ethical requirements of the profession. The principal responsibilities include: applying knowledge of nursing process, as it relates to scope of practice, to achieve identified patient goals when overall care needs are less complex, outcomes are predictable and risk of negative outcomes is low; identifies and recognizes abnormal or unusual patient responses and consults with the RN or other members of the health care team; provides aspects of care for highly complex patients when in close consultation with the registered nurse directing the patient's care; ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Nurses of Ontario and the UHN policy and procedure; demonstrating commitment to professional life-long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession; performing reflective practice, analyzing and evaluating nursing knowledge and modifying practice accordingly to improve the quality of nursing and patient care; performing cross-functional and other responsibilities, as assigned and/or requested. QUALIFICATIONS: - Completion of college, or other approved Registered Practical Nursing program including practicum experience - Current registration with the College of Nurses of Ontario as a Registered Practical Nurse - Current Basic Cardiac Life Support (B.C.L.S.) certification - Current medication certificate which meets the standards of practice established by the College of Nurses of Ontario - Demonstrates competencies as per unit/program competency-based checklist - Recent experience working with adult acute care Surgical patients preferred - Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards - Exercise initiative and good judgment with ability to multi-task - Able to comply with the physical demands of the Unit - Evidence of continuing education - Demonstrated leadership skills with evidence of strong organizational and decision making skills - Effective organizational, interpersonal and communication and time management skills - Knowledge of applicable legislative, UHN and/or departmental policies - Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand - Good computer skills Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
