About the role:

Calian Health is seeking Primary Care Nurse with interest in providing medical services to Canadian Armed Forces members in the military outpatient clinic located on Canadian Forces Base Meaford in Meaford, Ontario.

Your Key Duties:
  • Perform patient triage within the clinic using the principles of the Canadian Triage and Acuity Scale (CTAS)
  • Manage patient flow through the clinic to ensure patients are being seen in a timely manner based on the health care providers' scope of practice
  • Provide clinical guidance to clinic's Medical Technicians related to direct patient care and nursing interventions
  • Perform nursing clinical procedures such as health history and physical assessments, IV therapy, medication administration, dressing changes, blood pressure monitoring, specimen collection, and immunizations
  • Oversee the clinic schedules including each practitioner's schedule within the CDU to ensure efficiency and patient access
  • Provide administrative guidance to CDU clerks such as patient tracking and managing patient inquiries
  • Initiate collaborative practice meetings for patient care coordination within the clinic
  • Refer patients to other direct access health care providers to increase access to health services and improve patient outcomes
  • Initiate the short-term home care requirements for members with an expected return to work date of less than ninety days
  • Address patient complaints within CDU and forward any unresolved patient complaints to appropriate resource
  • Identify population health trends within the CDU such as infectious disease outbreaks
  • Provide patient counselling and teaching such as chronic disease management and post-operative management;
  • Organize and provide annual Infection Prevention and Control (IPAC) education for CF Health Services Centre staff to promote compliance with unit IPAC policies and Standard Operating Procedures (SOPs). The education topics include use of routine practices, chain of infection transmission, and additional precautions procedures for the proper donning and removal of personal protective equipment (PPE), Hand Hygiene; and Biomedical waste management.
  • Conduct annual hand hygiene audit and housekeeping audit IAW the current CF Health Services Group policy, Infection Prevention and Control in Canadian Forces Health Services Centres, provided by Task Manager;
  • Develop and distribute local IPAC and Community Health educational program for patients
  • Generate health surveillance data and trends by identifying the infections and infectious agents most common at the local level, conducting investigations of suspected healthcare acquired infections or disease outbreaks within the CF Health Services Centre, tracking infection rates and surgical site infection rates where applicable, and analyzing the data
  • Organize N95 mask fit-testing sessions with the Preventative Medicine Technician (P Med Tech) for clinic staff. Generate an annual status report to the Task Manager
  • Monitor, provide guidance and support clinic central sterilization processing staff on IPAC practices and maintain related records

The qualifications:
  • Diploma in Nursing or Bachelors Degree in Nursing
  • Registration and in Good standing with the College of Nurses of Ontario
  • 1-year full-time equivalent of experience, in family practice or ambulatory care environment or in a health care setting, such as tertiary care and community health centre
  • Current certification or willingness to obtain CPR Level C with AED

Type of Assignment:

We're seeking a Primary Care Nurse for a full-time, long-term contract position. Part-time availability (2-3 days) will be considered.

The clinic is open Monday to Friday from 7:30 am to 4pm. There is no evening or weekend work required.
