A day in the role as a Doctor's nurse at Ina Grafton Gage Home

As a Doctor's Nurse at Ina Grafton Gage Home (IGGH), you will report to the Director of Care and will be a part of our family like nursing team. As a Registered Nurse at IGGH you will provide care and satisfy the needs of residents at our home, which will reward you every day. This reward will come from the satisfaction you get from knowing you are making a large difference in someone's life.

Other responsibilities include, but are not limited to:

• Development and modification of the care plans in response to residents' change in status
• Liaises with other health care personnel, agencies and families
• Maintains good relationships with Medical/Support/Home, staff, residents, families and general public
• Supervises all activities on his/her unit, takes charge as required
• Monitors performance and completes performance evaluation for the PSW's
• Directs, assigns and evaluates the work of the PSW's
• May be in charge of Home as required. Utilizes leadership skills to identify problems and problem- solving on the unit
• Monitors that care delivered is consistent with the resident Bill of Rights.
• Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
• Required to participate in the improvement of the indicators and achieve satisfactory results.
• Ensures nursing supplies and equipment are properly and efficiently used
• Conducts unit rounds at the beginning and end of shift.
• Administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions
• Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home
• Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) and interdisciplinary team
• Completes assessments (RAI-MDS and other assessments) develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
• Addresses issues and concerns that may be communicated by the resident and/or family. Initiates the concern form and reports concerns/complaints to the Director of Care or Nurse Manager. Initiates immediate action(s) to resolve concern/complaint
• Conducts weekly MD rounds on designated days and time. Transcribes, processes and completes all physician orders, notifies physician of changes in resident's health status as required, and notifies substitute decision maker when there is a change in the resident's medication/treatment order
• Contacts pharmacy and troubleshoots as required
• Coordinates, books and leads care conferences. Conducts spot audits to ensure physicals are completed
• Works on the unit as a replacement for the unit supervisor as needed
• Ongoing monitoring and evaluation of the resident's skin and wound issues
• In tandem with the skin and wound coordinator the MD nurse would provide the nursing management with documentation/audits required for the Home's skin and wound program and analyze the data
• Conducts the Home's quarterly medication review

Qualifications

  • 1 to 2 years experience in long term care an asset
  • Current registration with CNO
  • Skilled in medication administration, treatment application, subcutaneous injections,reviewing charts and completing orders
  • Knowledge of Nursing Standards by CNO, Ministry of Health and Public Health
  • Strong assessment and critical thinking skills
  • Capable of supervising unregulated staff, ensuring the quality of care provided and that residents rights are upheld at all times
  • Knowledge of documentation requirements and care planning process, including computerized documentation
  • Excellent communication skills with residents, families and fellow employees
  • PCC experience is an asset
  • Must have a clear vulnerable sector screen, no older than 6 months, fully vaccinated (3 doses), TB test results less than 6 months old, evidence of flu shot, must be able to provide three work references.
